Fixed cutaneous sporotrichosis in a patient with numerous fungal elements

Abstract: Sporotrichosis generally shows no or a small number of fungal cells in tissue. Numerous fungal elements are usually associated with suppression of cellular immunity, either acquired or innate. The present case demonstrates that also topical immunosuppression can lead to increased fungal load at the affected site.

“…In most sporotrichosis cases, histopathological detection of the etiologic agent is difficult due to a low fungal load, with abundant cells being occasionally found in tissues only. 16 For this patient, we were unable to identify the fungal cells on the histological slides.…”

“…Sporotrichosis is a chronic, granulomatous, subcutaneous infection caused by dimorphic members of the genus Sporothrix, including at least six sibling phylogenetic species: S. pallida, S. brasiliensis, S. globosa, S. luriei, S. mexicana , and S. schenckii . 16 In China, S. globosa is predominant, 7–9 , 11 except for in the Nanchang area, where S. schenckii occurs along with S. globosa. 16 …”

“…The fungus is usually present only in small numbers. Consequently, isolating the fungus in culture is sometimes difficult, but it may be necessary to differentiate between various species as they respond differently to antifungal therapy [ 8 ]. Although the first treatment of choice is itraconazole, for cutaneous-lymphatic and cutaneous-fixed sporotrichosis, potassium iodide is reported to have a favorable response when administered in diluted solutions or drops of saturated solution [ 7 ].…”
