Background Hypertension is highly prevalent and associated with the elevated risks of cardiovascular diseases, dementia, and physical disabilities among adults. Although the correlation between bilirubin and hypertension has been reported, the observation in quinquagenarian population is scarce. We aimed to examine bilirubin-hypertension association in Guankou Ageing Cohort Study. Methods Participants ≥ 55 years were recruited and their questionnaires and physical examination data were collected. Kaplan–Meier survival analysis and Cox proportional hazards regression were implemented to assess the hypertension risk. The non-liner dose–response relationships of bilirubin-hypertension were determined by restricted cubic spline (RCS) models. Receiver operating characteristic (ROC) curves and multiple factors analysis (MFA) were performed to evaluate the predictive abilities. Results 1881 eligible participants (male 43.75%, female 56.25%) with the median age of 61.00 (59.00–66.00) were included. The hazard ratio (HR, 95% CI) of serum total bilirubin (STB) and unconjugated bilirubin (UCB) were 1.03 (1.01–1.05) and 1.05 (1.03–1.07), while conjugated bilirubin (CB) showed a weak protective effect with the HR of 0.96 (0.92–0.99), and the associations remained significant in all models. RCS analyses further indicated the similar bidirectional effects of STB and UCB with the cut-off of 12.17 μmol/L and 8.59 μmol/L, while CB exhibited inverse bidirectional dose–response relationship with a cut-off of 3.47 μmol/L. ROC curves and MFA showed baseline STB combined with age, BMI, and waist circumference could well discriminate the low and high of hypertension risk. Conclusions Our findings suggested the higher levels of total and unconjugated bilirubin were hazardous factors of hypertension, while an inverse effect presented when more bilirubin was conjugated.

BACKGROUND: Some disinfection by-products (DBPs) are reproductive and developmental toxicants in laboratory animals. However, studies of trimester-specific DBP exposure on adverse birth outcomes in humans are inconsistent. OBJECTIVE: We examined whether trimester-specific blood and urinary biomarkers of DBP were associated with small for gestational age (SGA), low birth weight (LBW), and preterm birth. METHODS: A total of 4,086 blood and 3,951 urine samples were collected across pregnancy trimesters among 1,660 mothers from Xiaogan City, China. Blood samples were quantified for biomarkers of trihalomethanes (THMs): chloroform (TCM), bromodichloromethane, dibromochloromethane, and bromoform. Urine samples were quantified for biomarkers of haloacetic acids (HAA): dichloroacetic acid and trichloroacetic acid. Birth outcomes were abstracted at delivery from medical records. We used Poisson regression models with log link functions to estimate risk ratios (RRs) and 95% confidence intervals (CIs) for SGA, LBW, and preterm birth across tertiles (or categories) of DBP biomarker concentrations measured across pregnancy trimesters. We also examined the relative exposure differences across gestation comparing adverse outcomes with normal births using mixed-effects models. RESULTS: Blood TCM concentrations in the second trimester were associated with an elevated risk of SGA comparing middle vs. lowest (RR, 2.34; 95% CI: 1.02, 5.35) and highest vs. lowest (RR, 2.47; 95% CI: 1.09, 5.58) exposure groups. Third-trimester blood TCM concentrations were also associated with an increased risk of SGA comparing the second tertile with the first (RR, 2.61; 95% CI: 1.15, 5.92). We found that maternal blood TCM concentrations were significantly higher for SGA compared with non-SGA births across the period from 23 to 34 wk gestation. Other blood and urinary DBP biomarkers examined were unrelated to SGA, LBW, or preterm birth. CONCLUSION: Blood TCM concentrations in mid to late pregnancy were associated with an increased risk of SGA, whereas other biomarkers of DBPs examined across pregnancy were not associated with birth outcomes.

Trihalomethanes containing bromine (Br-THMs) are compounds formed from the reaction of organic and inorganic substances in drinking water with chlorine as a common disinfectant. Epidemiological evidence indicated various outcomes as a result of long-term exposure to these compounds. This study aims to investigate the concentration of urinary Br-THMs in a sample of third-trimester pregnant women and to determine its correlation with lifestyle factors. This cross-sectional study was conducted among 118 pregnant women in Isfahan, Iran. The third-trimester urine Br-THMs including dibromochloromethane (DBCM), bromodichloromethane (BDCM), and broomcorn (TBM) were measured using solid-phase micro-extraction gas chromatography. The distribution of urinary Br-THMs concentration was positively skewed then these variables were subjected to logarithmic transformation. Spearman ranks correlation coefficient, and two independent samples t-test were used to assess the association of urinary Br-THMs concentration with its demographic and lifestyle determinants.The findings showed that the mean (SD) concentrations of DBCM, BDCM, TBM and brominatede THMs were 14.76 (27.17), 10.25 (11.37), 4.96 (3.21) and 30.17 (40.80) ng/L, respectively. In addition, significant inverse correlation was found between the use of water storage tanks and Br-THMs (DBCM, BDCM, TBM and brominatede THMs, P-value = 0.005, 0.019, >0.001 and 0.007 < 0.05, respectively).In this study, among the quantitative and qualitative variables, the use of storage tanks had an inverse correlation with Br-THMs. In addition, the status of urinary concentration of Br-THMs in pregnant women was less than in the other countries in the same area. Improving water quality and using modern methods to eliminate disinfection by-products as well as improving lifestyle patterns are efficient methods to reduce exposure to these contaminants.
