First Place: Renal Oxygenation During Robot-Assisted Laparoscopic Partial Nephrectomy: Characterization Using Laparoscopic Digital Light Processing Hyperspectral Imaging

Abstract: The laparoscopic HSI system successfully characterized dynamic changes in renal oxygenation during RALPN. Intraoperative laparoscopic HSI outcomes have the potential to predict postoperative individual kidney function.

“…Tumor-specific arterial branches are microdissected and superselectively clamped with micro-bulldog clips. Selective arterial control is confirmed intraoperatively by color Doppler sonography [69,70], hyperspectral imaging [71,72], or robotic vascular fluorescence imaging [73]. In an initial study of 15 patients, there was no change in estimated GFR (eGFR) [66].…”

“…Furthermore, younger patients had a lower median baseline %HbO 2 and a signifi cantly higher median preoperative eGFR, perhaps refl ective of age-related decline in renal oxygen consumption. No signifi cant associations between %HbO 2 and diabetes, hypertension, and smoking history were observed [ 25 ].…”

“…ing open partial nephrectomy, demonstrating that in the artery+vein group, a lower mean ischemic %HbO 2 was associated with signifi cantly lower eGFR at most recent follow-up on both univariate and multivariate analysis [ 24 ]. Most recently, Olweny et al used a novel laparoscopic DLP ® -HSI system to study renal oxygenation in patients undergoing robotic-assisted laparoscopic partial nephrectomy [ 25 ]. They demonstrated a significant inverse association between baseline %HbO 2 and baseline eGFR as well as between baseline %HbO 2 and eGFR at most recent follow-up ( p < 0.05).…”
