First Pass Effect in Patients Treated With the Trevo Stent-Retriever: A TRACK Registry Study Analysis

Abstract: Results: The rate of FPE in TRACK was 23% (140/609). There was no association between patient demographics and FPE, including age (p = 0.36), sex (p = 0.50), race (p = 0.50), location of occlusion (p = 0.26), baseline NIHSS (p = 0.62), or past medical history. There was no difference in the use of a balloon-guide catheter or general anesthesia (49 and 57% with FPE vs. 47 and 64%, p = 0.63 and p = 0.14, respectively). Clinical outcomes were significantly associated with FPE; 63 vs. 44% in non-FPE patients achie… Show more

“…Use of BGC was reported to be a predictor of FP (6, 30). The use of a BGC did not affect angiographic outcomes in other reported studies (29,31). We achieved good FP results without the use of a BGC.…”

“…FP frequency (TICI ≥2c) described for stent retriever in M1 occlusions was 39.7% (Trevo, almost in 90% of cases) ( 9 ). In the data of the NASA registry, for M1 occlusions, FP was 29% (Solitaire) ( 6 ), with younger and selected patients, in 2012 and 2013, and in the TRACK registry, FP was 22.3% for M1 occlusions (Trevo 4 × 20) ( 29 ). In the ASTER trial, FP in M1 occlusions was 31.2% with stent retriever or aspiration, within 6 h of onset ( 10 ).…”

“…Thus, identifying factors influencing FPE could help clinicians and interventionalists maximize the benefit of MT through suitable patient selection and pre-interventional risk modification. There are many studies seeking to explore this phenomenon, but with inconsistent results (7,8,11,(13)(14)(15)(16)(17)(18)(19)(20). For example, balloon guide catheters (BGC) and non-internal carotid artery (ICA) terminus occlusion were correlated with FPE in the study of Zaidat et al (7), but factors such as older age, a lower systolic blood pressure, and conscious sedation were not (17).…”

“…Successful first pass rates range from 24% to 29%. 16,[30][31][32][33][34] Therefore, improvement in techniques and devices is still needed to increase these rates, even if the thrombi due to dissection or intracranial atherosclerosis are excluded.…”
