First known case of Bartonella quintana endocarditis in Sweden

Abstract: In this report, we present the first known case of Bartonella endocarditis in Sweden. IgG antibody titres to Bartonella spp. were elevated but blood cultures remained negative. Sequencing of a gltA fragment from DNA extracted from heart valve tissue specimens revealed sequence homology with B. quintana.

“…or Coxiella burnetii, which have been reported at centres in Southern Europe 8 but not from Northern Europe 22 ; only one case of Bartonella endocarditis has previously been reported from Sweden. 23 The valve culture yield was low in this study, in only 16% the results were concordant with blood cultures or PCR. It should be questioned if this method it should be recommended when 16S rDNA analysis is available.…”

“…Only one episode of IE that was caused by C. burnetii ( 20 ) and one episode of IE that was caused by Bartonella spp. ( 22 ) have been reported in Sweden, and we found no cases of either in our cohort. Two episodes of T. whipplei IE were diagnosed through 16S-analyses and, for these patients, the treatment was much affected by this microbiological finding.…”

“…In this report, the positive titre for anti- B. quintana antibodies (1 : 128) was unexpected, since clinical infections with the bacteria are uncommon [17, 18] and it has not been associated with cat bites previously in Sweden [19, 20]. Infections caused by B. quintana are exclusively seen in patients with actual or recent contact with body lice [21].…”
