First-Episode ‘Coenesthetic’ Schizophrenia Presenting with Alien Hand Syndrome and Partial Agenesis of the Corpus Callosum

Abstract: We describe the case of a 23-year-old Caucasian woman who presented with alien hand syndrome and a first episode of the coenesthetic subtype of schizophrenia. ‘Alienness’ of her non-dominant hand was intimately phenomenologically associated with the onset of first-psychosis. Cerebral MRI revealed a partial agenesis of the corpus callosum with a complete absence of the rostrum, hypoplastic anterior and inferior genu, and a hypoplastic splenium. This case suggests that this syndrome can occur with the developmen… Show more

“…To our knowledge, two case studies have been published examining WM abilities in AgCC, both adults. However, results are contradictory, with impaired performance on a 2-back task in one case (Simon et al, 2008), and average performance on auditory-verbal and visual WM tasks in the second case (Reddy et al, 2010). In addition, Sauerwein and Lassonde (1994) reported working memory performance below the average range but not significantly different from the control group in 9 individuals with AgCC from 10 to 29 year-old.…”

“…We found several clinical cases reporting associations between lesions of the corpus callosum and psychotic manifestations that illustrate the pathophysiologic involvement of this structure in secondary psychosis (Figure 3, Supplementary Table 3). In complete or partial agenesis of the corpus callosum (Bhatia 2016;Simon et al 2008), or in X-linked adrenoleukodystrophy (Smith, Williams, and Misra 2018), delusions and hallucinations may be in the foreground, sometimes without association with neurological symptomatology. Corpus callosum's pathogenicity in psychosis is also highlighted by the description of a reversible splenial lesion in a teenager treated for a nephrotic syndrome.…”

“…The CC plays the main role in integrating information from both hemispheres; it controls thought as well as emotional and behavioral processes [ 24 ]. The participation of the CC in the pathogenesis of psychosis is indicated by reports of cases of psychotic symptoms when partial or complete agenesis of the CC has occurred [ 25 ].…”
