Fine needle aspiration of hepatic angiomyolipoma with extramedullary hematopoiesis: A case report

Abstract: Angiomyolipoma (AML), a perivascular epithelioid cell tumour, is a benign tumour, typically involving the kidney. In this location, the cytological and histological characteristics of this lesion allow for a straightforward diagnosis in most cases. When AML occurs in extrarenal sites such as the liver, AML can mimic a malignant tumour clinically and radiologically. 1 A definitive diagnosis of hepatic angiomyolipoma (HAML) usually relies on histopathological evaluation of biopsy or resection material. A diagnos… Show more

“…Other rare liver tumours with rhabdoid morphology may need to be differentiated from HCC as they carry dismal prognosis 27 . Extramedullary haematopoiesis is a helpful diagnostic feature of HB 28 but can be found commonly in the paediatric age group and also in other hepatic neoplasms like HCC, 29 infantile haemangioendothelioma, angiosarcoma, or angiomyolipoma 30 …”

“…Abdominal ultrasound is initial and simple examination for diagnosing hepatic masses, which is well circumscribed and homogenous because contains many fatty components with rapid enhancement in the arterial phase of ultrasound contrast and distinguish from hemangiomas with a posterior echogenic image [12] . Endoscopic ultrasound guided fine-needle aspiration (EUS-FNA) was rarely used in selective case for diagnosing, due to component of blood vessels could led to internal tumoral hemorrhage or rupture [13] . In computed tomography ( CT ), the tumor was hypodense due to adipose component, hypervascular in richly vascularized HAML with wash-out in the portal and late portal phase, or persistent portal and late-phase enhancement in HAML with few or no vessels [14] .…”

“…Angiomyolipoma (AML) is a benign, solid, mesenchymal tumor and is one of the perivascular epithelioid cell neoplasms (PEComas) [ 1 - 2 ]. It is the most common benign renal tumor but rarely encountered at extrarenal sites, the liver being the second commonest site [ 3 - 4 ], and is usually an incidental imaging finding [ 5 ]. AMLs most commonly involve the uterus within the female genital tract [ 6 ].…”
