Fine‐needle aspiration cytology of mesenchymal tumors of the breast

Abstract: This report describes the fine-needle aspiration (FNA) cytologic findings of 15 cases of sarcomas involving the breast out of a combined series of 2,064 breast FNA biopsies, including 580 malignancies, thereby accounting for 2.6% of all the malignant breast tumors. The series consisted of 14 women and one man with a mean age of 48.4 yr (range, 29-63). There were eight cases of cystosarcoma phyllodes, including one malignant cystosarcoma phyllodes. “…Although there have been reports of metastatic PNET involving the breast [8,9], a Medline search (1976-2006) revealed only 1 publication on PNET of the breast as a primary tumor [7]. According to our experience with the current patient, the clinical findings first resembled those of a hematoma, which however increased in size.…”

“…The term PNET includes malignant small round cell tumours of the thoracopulmonary region (Askin's tumour), Extra skeletal Ewing's sarcoma, peripheral neuroblastoma, and peripheral epithelioma. 12 The cell of origin of this tumour is uncertain. Originally, it was thought that EWS / PNET might arise from the neuroectoderm, but recent data have suggested that this tumour is more likely to originate from primitive stem cells and that the degree of malignancy appears to depend upon the stage of stem cell arrest during differentiation.…”

“…The pleomorphic and bizarre cellular features can suggest the diagnosis of metaplastic and pure sarcomas of the breast. The establishment of a diagnosis of sarcoma is generally difficult by FNA biopsy [5,6,8].…”
