Objective: Fine-needle aspiration biopsy (FNAB) is widely used in the management of salivary gland masses. Its main advantage is its ability to differentiate benign from malignant disease. In this study, we aimed to evaluate the diagnostic ability of FNAB in salivary gland masses. Methods:The records of patients who had undergone FNAB before parotidectomy or submandibular gland excision between 2005 and 2013 were retrospectively analyzed. FNAB results were classified as negative, positive, suspicious for malignancy, and non-diagnostic. Preoperative FNAB results were compared with definitive histopathological results. The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of FNAB results were calculated.Results: A total of 285 patients were enrolled. Among them, 230 (80.7%) had parotid gland and 55 (19.3%) had submandibular gland masses. Following a definitive histopathological examination, the most common benign tumor was pleomorphic adenoma (52.6%), whereas malignant tumors were mucoepidermoid carcinoma (2%) and squamous cell carcinoma (2%). The sensitivity, specificity, PPV, NPV and accuracy of FNAB results were 76.9%, 95.4%, 75%, 95.9%, and 92.6%, respectively. The rate of a suspicious cytology was 5.2% (15 patients) and that of a non-diagnostic cytology was 8.8% (25 patients). Conclusion:FNAB is a safe and simple diagnostic tool for the diagnosis of salivary gland masses and has a relatively high sensitivity and specificity. It may provide valuable information for patient counselling and surgical planning. The major drawbacks include a lower sensitivity than specificity and a relatively high rate of non-diagnostic results.

AIM: To determine the diagnostic value of fi ne needle aspiration biopsy (FNAB) of parotid masses in the basis of postoperative histopathological results. METHODS: The study was conducted in Ankara Atatürk GirișTükürük bezi tümörleri, tüm baş boyun tümörlerinin %3-12'sini ve tüm vücut tümörlerinin ise %2-3'ünü oluşturmaktadır. Bu tümörlerin %80'i parotis bezinden köken almaktadır 1 . Parotis bezi tümörlerinin yaklaşık %80'i benign, %20'si ise malign yapıdadır 1-3 . Parotis tümörlerinde preoperatif değerlendirme, parotise komşu yapılardan köken alan lezyonların ayrılmasında ve lezyonlara yönelik farklı tedavi yaklaşımları olması nedeniyle oldukça değerlidir. Doğru değerlendirme ile medikal tedavi, klinik takip ya da cerrahi tedavi seçeneklerinden birine karar verilmesi yanı sıra, preoperatif değerlendirme bezin fasial sinir ile yakın komşuluğu nedeniyle cerrahi planlama ve gereksiz cerrahiyi engellemek açısından da önemlidir 4,5 . Preoperatif patolojinin bilinmesi cerrahi planlamada da kolaylık sağlamaktadır. Operasyon öncesi değer-lendirme aşamasında ince iğne aspirasyon biyopsisi (İİAB) sıklıkla kullanılmaktadır.Bu çalışmada, parotis bezi kitlelerinin histopatolojik tanıları ile operasyon öncesi yapılan İİAB bulguları karşılaştırılarak İİAB'nin duyarlılık, özgüllük, pozitif tahmin değeri (PPV) ve negatif tahmin değerlerinin (NPV) ve etkinliğinin belirlenmesi amaçlandı.

Based on the literature reviews summarize the diagnostic value of fine needle and core needle biopsy in salivary gland masses, revealing the advantages and disadvantages of these two methods. Salivary gland tumors are diagnostic challenge in a clinical practice and an accurate diagnosis is essential for its adequate management. Fine needle aspiration cytology and core needle aspiration cytology are a useful techniques for evaluating masses suspected of being of salivary gland origin. The reported diagnostic accuracy of FNAC can be as high as 98% when adequate material is obtained, but the rate of insufficient or non-diagnostic rate is up to 29%. Core needle biopsy aids in differentiating malignant from benign masses with reported sensitivities of 75~89%, specificities of 96.6~100%, and accuracies of 91.9~100%. Conclusions: Studies have shown that fine needle and core needle aspiration cytology have high sensitivity and specificity for salivary gland lesions. In the absence of ultrasound-guidance, or on-site cytopathologist, the accuracy of these methods often falls off dramatically.
