“…The Guidance for Implementation of the IIRMS Specification (updated in September 2018 and again April 2020) promotes adherence to four main theoretical domains; attachment theory, desistance and ‘Good Lives’ model, risk needs responsivity and theory of needs. IIRMS also recognises that individuals on the OPD pathway must have their basic needs met if they are to find the emotional space and containment needed to enact change (OPD Pathway IIRMS Specification, 2018; 2020; Ramsden et al, 2016). It is also acknowledged that development of functional and containing therapeutic relationships is intrinsic within effective service provision (Ryan et al, 2019).…”

“…Although the pathway continues to evolve, we recognise that the OPD pathway as a system and us as practitioners must constantly strive to learn and improve. Preliminary evaluations have explored the barriers to engagement with IIRMS (Webster and Gardner, 2020) and have highlighted some of the particular dynamics involved when working with women on IIRMS (Ramsden et al, 2016). Nevertheless, as clinical leads of three of the longest established IIRM services, we recognise that there is a lack of research in this field and, although data-driven research is in train, we are now reflecting more broadly on who we "need to do better" with, what seems to work and with whom.…”
