Fifteen years of infrapopliteal arterial reconstructions with cryopreserved venous allografts for limb salvage

Abstract: Objective: The availability of autologous vein grafts remains the limiting factor in infragenual bypass surgery in many patients with critical limb ischemia (CLI). Alternatives such as prosthetic conduits are known to have a poor outcome and most are not resistant to infection. Based on previous experimental work, we started to use cryopreserved saphenous vein allografts for this indication 15 years ago. To evaluate their outcome, we performed a retrospective study of those bypasses with distal anastomosis on … Show more

“…The initial phase in the non-injury setting of venous bypass NH is probably due to EC dysfunction and eventual denudation (Johnson et al 2000;Randon et al 2010). The role of chronic inflammation, however, remains unclear, but is not supported by our results.…”

“…Although Pupka and coworkers in their response to that comment without providing evidence speculated that AB0-bloodgroup compatibility is required it must be noted that this highly important immunological aspect has recently been investigated by Della Schiava and co-workers which did not confirm this assumption [35]. In accordance with tentative indications by earlier studies [36,37] the authors provided evidence that (in-)compatibilities of AB0-as well as Rhesus antigens did not lead to altered outcomes regarding death, thrombosis, rupture, stenosis, aneurysmal degeneration and follow up patency over a five years' follow up period thus pointing on a minor relevance of classical blood group antigens in this regard. Della Schiava and co-workers consequently speculated that incompatible HLA-antigens may be the main immune barrier which leads to the observed phenomenon of long term immune-mediated arterial allograft degeneration [35].…”
