Field Validation of the Los Angeles Motor Scale as a Tool for Paramedic Assessment of Stroke Severity

Abstract: Background and Purpose-The Los Angeles Motor Scale (LAMS) is a 3-item, 0-to 10-point motor stroke-deficit scale developed for prehospital use. We assessed the convergent, divergent, and predictive validity of the LAMS when performed by paramedics in the field at multiple sites in a large and diverse geographic region. Methods-We analyzed early assessment and outcome data prospectively gathered in the FAST-MAG trial (Field Administration of Stroke Therapy-Magnesium phase 3) among patients with acute cerebrovasc… Show more

“…Identification is the crucial first step in getting the right patient to the right treatment more quickly and, as the outcome depends on time to reperfusion, might improve outcomes 9. About 20 pre-hospital scales exist16; some of the most common scales used are the Los Angeles Motor Scale (LAMS),17 Cincinnati Prehospital Stroke Severity Scale (CPSS),18 and Rapid Arterial Occlusion Evaluation Scale (RACE) 19. Many of the scales were designed initially to identify patients with stroke as opposed to conditions that mimic stroke, but some were specifically designed for identification of patients with stroke with LVO (for example, Vision, Aphasia, Neglect or VAN) 20.…”

“…Although a variety of prehospital stroke severity scores have been derived, few have been validated in the field 7. The Los Angeles Motor Scale (LAMS) score has shown very good validity8–10 and was selected for use in our region 11…”

“…To reduce the incidence of overtriage, several stroke scoring systems have been developed to assist EMS providers with accurate diagnosis. 16,17 Existing literature affirms that the ideal tool has yet to emerge. 3,18 The Smith (2018) et al meta-analysis demonstrated that LVO-specific triage schemes failed to perform better than less-selective tools.…”
