Field-testing of the ICHD-3 beta/proposed ICD-11 diagnostic criteria for migraine with aura

Abstract: The new criteria in ICHD-3 beta/proposed ICD-11 for 1.2 MA, 1.2.1 MTA, 1.2.3.1 FHM and 1.2.3.2 SHM have more desirable properties than ICHD-2 and the ICHD-3 beta alternative criteria. The criteria for 1.2.2 migraine with brainstem aura should be more restrictive.

“…The rate of the probable diagnosis can be altered if its diagnostic criteria are modified, which may be appropriate because this diagnosis is sometimes considered to be a neglected subtype of PHD that has a similar clinical burden and common therapeutic responses as the other headaches diagnoses, such as a probable migraine . However, the absence of a single criterion in a history‐based clinical diagnosis also increases the risk of diagnostic uncertainty or a misdiagnosis.…”

“…The reported diagnostic rates for migraine and tension-type headaches are as low as 40%, which may be attributed to under-application of ICHD criteria ( 14 ). While there have been some papers about field-testing in each category of headache disorders after publication of the ICHD-3β, the systematic application of the entire ICHD-3β has not been assessed yet ( 12 15 16 17 18 19 20 21 ).…”
