Fibromyalgia (FM) is a highly prevalent chronic disease. About 4.7% of the world's population suffers from generalized pain and hypersensitivity, in addition to a wide range of physical and psychological symptoms. The etiopathogenesis of this disease is multifactorial, which makes its diagnosis and treatment challenging. Recently, the increase in the number of studies on microbiota has provided new data that can help to understand the onset and development of FM. An updated systematic review of the causes of FM has been carried out in this work. Particularly in the last decade, research has focused on the gut–brain axis, which has emerged as a crucial mechanism for microbiota‐host crosstalk. In FM patients, quantitative imbalances of the intestinal microbiota (dysbiosis) and bacterial metabolites with differential relative abundance have been found, especially short‐chain fatty acids and lipopolysaccharides. Furthermore, the microbiota has been found to indirectly influence host neurotransmitter mechanisms, mainly through the serotonin precursor, glutamate, and gamma‐aminobutyric acid. Thus, all these mechanisms and their influence on the etiopathogenesis of FM are discussed in this review.

One of the challenges faced by behavioral scientists is the lack of modeling methodologies for accurately determining when a behavior becomes problematic. The authors propose “behavioral posology” as a novel modeling paradigm for quantifying the healthy limits of behaviors through the concept of behavioral dose. As an example of this paradigm, a pharmacokinetic/pharmacodynamic model of a hypothetical digital behavior is presented, based on opponent process theory. The generic model can be adapted to simulate Solomon and Corbit's model of affective dynamics from 1974, and the model predicts features of addiction such as hedonic allostasis, withdrawal, and apparent tolerance. A behavioral frequency response analysis (BFRA) of the model demonstrates how behavior repetition may result in a hormetic dose–response relationship that depends on the frequency of the behavior. The model can be experimentally validated using Ecological Momentary Assessment, allowing researchers to hypothesize, model, and test causal mechanisms for behavioral addictions. The potential for behavioral posology to be applied as a clinical support tool in psychological medicine is discussed, as this modeling framework may help to detect and limit behaviors being performed too frequently based on factors such as the person's moral beliefs.

ObjectivesTo evaluate the risk of all‐cause, specific mortality among patients with fibromyalgia, which is a controversial topic.MethodsWe conducted a thorough search for cohort studies across the PubMed, Cochrane Library, and Embase databases, from their inception to 1 March 2023, using medical subject headings and relevant keywords. All data were meticulously analyzed using Stata statistical software version 16.0. The protocol was registered on PROSPERO (CRD42023402337).ResultsAfter analyzing seven cohort studies involving 152 933 individuals published between 2001 and 2020, we found no clear evidence linking fibromyalgia or widespread pain to all‐cause mortality risk (odds ratio [OR] 1.11, 95% confidence interval [CI] 0.81–1.53; I2 = 82.6%, p = .505). However, our subgroup analysis revealed that the risk of suicide was significantly higher in fibromyalgia patients compared with non‐fibromyalgia patients (OR 5.39, 95% CI 2.16–13.43; I2 = 69.9%, p < .05).ConclusionsOur research did not discover any proof indicating a link between fibromyalgia or widespread pain and all‐cause mortality. However, it is worth noting that there may be a potential correlation between individuals with fibromyalgia or widespread pain and a higher likelihood of suicide. As we had a limited number of participants in our study, further research is necessary to thoroughly investigate the relationship between these factors.
