Fibrinogen and factor VII in the prediction of coronary risk. Results from the PROCAM study in healthy men.

Abstract: Coronary thrombosis is regarded as the final occlusive event in the progress of coronary heart disease (CHD). Disturbances of the hemostatic system may favor this process and thus may indicate increased risk of myocardial infarction. Coagulation and lipid factors were measured in 2116 healthy male participants of the Prospective Cardiovascular Miinster (PROCAM) study. After 6 years of follow-up, 82 coronary events (9 sudden cardiac deaths and 14 fatal and 59 nonfatal myocardial infarctions) were observed. The … Show more

“…The fact that HDL cholesterol and triglyceride were unaltered is probably because most patients were on lipidlowering drugs and additional weight loss was insufficient to affect any changes. Additionally, weight loss was completed at 12 weeks and insufficient time was available for weight stability to encourage a rise in HDL (Heinrich et al, 1993). The significant decrease in factor VII activity achieved in the present study supports the findings of other studies where both moderate weight loss (Rissanen et al, 2001,Folsom et al, 1993 and much greater weight loss (18%) have been achieved .…”

“…1995). Other risk factors that are elevated by overweight include hypertension (Ramsey et al, 1978), blood lipids and coagulation factors (Williamson et al, 1995;Heinrich et al, 1993). All improve with loss of 5 -10% or 5 -10 kg body weight (Goldstein, 1992).…”

“…A high plasma ®brinogen concentration is a risk factor of CVD in healthy men (Wilhelmsen et al, 1994;Heinrich et al, 1994). Fibrinogen is an acute-phase protein and we therefore speculate whether the increased ®brinogen concentration found in the G-group may be attributed to immunostimulation.…”

“…Fibrinogen levels in Japanese are lower than in Caucasians (Iso et al, 1993) and the same appears to be the case in Hong Kong Chinese (EDJ, unpublished observations). In the German PROCAM Study individuals with low ®brinogen levels developed less coronary heart disease at follow up even if their LDL cholesterol concentrations were high (Heinrich et al, 1994). Thus low ®brinogen levels are a possible protective factor in the local population.…”
