Fibrin glue in the management of anal fistulae

Abstract: Fibrin glue is simple to use, has a minimal morbidity and should not affect later treatment options in the event of its failure. It is therefore theoretically attractive as a first line treatment in the management of those types of anal fistula in which it has been shown to work. However, further research into 'biological' glues is merited and these subject to randomised controlled study.

“…It had been suggested in the past, that suturing of the internal opening prevented the expulsion of the clot, but various recent trials [10][11][12][13][14][15] have proved that there was no significant change in the healing rate, with this procedure. Nine out of these 10 recurrences (90%), were reported in the first 3 months of the follow up.…”

“…This treatment modality is appealing because it carries no risk of incontinence associated with division of sphincter muscles. It is safe and easy to perform, avoids open perineal wounds, and does not interfere with future surgical treatments if necessary (10,14) . The efficacy of the fibrin glue injection, however, remains in question.…”

“…The procedure is very easy to perform, has no significant morbidity and preserves continence without compromising future surgical treatments (6) . However, the success rates of the fibrin glue injection have range from 0%-100% (10) . Such a wide variation seems to result from differences in the etiology and complexity of the fistulas included in each series, as well as from variable sample sizes and duration of patient follow-up (10,20) .…”

“…Apesar dessas discrepâncias metodológicas, pode-se concluir que o método é de fácil execução, com mínimas complicações e não afeta as opções de tratamento posteriores, em caso de insucesso do método. Pode ser, portanto, uma opção atrativa como primeiro método de tratamento de fístulas anais 4 .…”
