Atrial fibrillation is the most common cardiac arrhythmia in the world, with an estimated prevalence of 2 to 4%, and represents a high burden of disease worldwide. It is defined as a supraventricular tachyarrhythmia with loss of atrial activity, diagnosed by specific electrocardiographic findings. Its classification is clearly defined, based on the form of presentation, duration and the resolution of the episodes. One of the pillars in the treatment of this pathology is anticoagulant therapy, which represents a challenge due to the heterogeneity of the patients, however there are specific situations such as chronic kidney disease, oncological diseases and liver disease, where the situation becomes even more complex, however different studies have been carried out in which the best strategies based on scientific evidence have been proposed for the approach of these patients in special situations.

Atrial Fibrillation (AF) is the most common cardiac arrhythmia worldwide. It is associated with reduced quality of life and increases the risk of stroke and myocardial infarction. Unfortunately, many cases of AF are asymptomatic and undiagnosed, which increases the risk for the patients. Due to its paroxysmal nature, the detection of AF requires the evaluation, by a cardiologist, of long-term ECG signals. In Colombia, it is difficult to have access to an early diagnosis of AF because of the associated costs to the detection and the geographical distribution of cardiologists. This work is part of a macro project that aims to develop a specific-patient portable device for the detection of AF. This device will be based on a Convolutional Neural Network (CNN). We aim to find a suitable CNN model, which later could be implemented in hardware. Diverse techniques were applied to improve the answer regarding accuracy, sensitivity, specificity, and precision. The final model achieves an accuracy of , a specificity of , a sensitivity of and a precision of . During the development of the model, the computational cost and memory resources were taking into account in order to obtain an efficient hardware model in a future implementation of the device.

La fibrilación auricular (FA) es la arritmia recurrente más habitual en la práctica clínica. Su prevalencia se multiplica con la edad y es un factor de riesgo directo para importantes enfermedades cardiovasculares. Basado en ello, se tiene que el objetivo de este ensayo científico fue describir el manejo de la fibrilación auricular, según sus patologías de base más frecuentes. Se enmarcó en una metodología de tipo documental bibliográfica. Como técnica principal de recopilación de información se indagó sobre el tema en documentos en idioma español en las bases de datos contempladas en la biblioteca virtual de salud de Google Académico, Scielo, Dialnet, Elsevier, otras. Para ello, se utilizó la búsqueda avanzada a través de descriptores clave como: fibrilación auricular, patología de base, manejo. De este modo, se analizaron de manera general las diferentes patologías y factores de riesgo asociadas a la FA; como forma de fortalecer los conocimientos sobre el tema que permita manejar de manera adecuada esta alarmante enfermedad. Las fuentes consultadas indican que en los países desarrollados, la prevalencia de la FA es aproximadamente del 1,5-2% de la población general, afecta al 6% de los mayores de 60 años y al 8% de los mayores de 80 años, aqueja más a hombres que mujeres, y que se espera que su incidencia aumente en los años futuros, incluidos en estos pronósticos los países latinoamericanos. Dentro de las conclusiones más relevantes se subraya la cuestión de que la FA, aumenta la morbilidad y mortalidad conducente en muchos casos a situaciones discapacitantes, por tanto, se hace necesario que el personal especializado realice un diagnóstico, atención y tratamiento óptimo; a fin de minimizar las posibles complicaciones, teniendo en cuenta el control de ritmo/respuesta ventricular y la anticoagulación oral como los principales pilares en su manejo.
