Fetal mediastinal teratoma: Misinterpretation as congenital cystic lesions of the lung on prenatal ultrasound

Abstract: Congenital mediastinal teratoma can lead to development of hydrops fetalis and may be misinterpreted on ultrasound. In this case report, ultrasound revealed severe fetoplacental hydrops, moderate posthemorrhagic hydrocephalus, and multiple pulmonary cysts suggesting cystic adenomatoid malformation and displacement of the heart to the left side. Autopsy of the hydropic 24-weeks male fetus showed a large cystic-solid mediastinal mass that was consistent with nonmetastatic immature teratoma. It also demonstrated … Show more

“…Prenatal US diagnosis of mediastinal teratoma has been scarcely reported, especially in the second trimester, and is summarized in Table 1 [2,[7][8][9]13,14,[18][19][20][21][22][23][24]. In our case, a mediastinal mass was detected at US at 21 wga + 1 day, being the second fetal case reported both with an early diagnosis and without hydrops.…”

“…43 Fetal mediastinal teratomas often cause second trimester nonimmune hydrops, therefore prognosis is rather poor. [44][45][46][47][48][49][50] In our series, post-mortem histological examination identified immature mediastinal teratoma in one case, which is more rare, tend to grow faster and thus lead to hydrops quicker. 48 In case of smaller tumors or lacking compression of mediastinal organs, prognosis might be better.…”
