Fertility-Sparing and Less Radical Surgery for Cervical Cancer

Abstract: Purpose of Review Patients with early-stage cervical cancer who desire future fertility may be candidates for less radical surgery. We review the literature supporting this approach in early-stage disease. Recent Findings Retrospective data have shown that in carefully selected patients, the risk of parametrial involvement is less than 1%. This has led to interest in moving away from radical surgery towards more conservative approaches. Data from the newly… Show more

“…The fertility-sparing measures, including hormonal treatment, hysteroscopic resection [ 70 ], gametes cryopreservation [ 71 ], should be appropriately reserved in the treatment of early-stage or low-risk endometrial cancer (EC) [ 70 , 72 , 73 ], cervical cancer (CC) [ 74 ] and OC [ 75 ], and the non-coding RNA-based diagnostics and therapeutics are the valuable options for implications for the fertility-sparing process [ 76 ]. Some lncRNAs have also been reported to be involved in the anti-EC effects of progesterone, which may provide new insights into fertility-sparing process [ 77 ].…”

“…Three pregnancies occurred among 23 patients (13%), all three resulting three live births (100%). Overall, the data currently available in the literature are too limited to make statements regarding the feasibility and reproductive outcomes of conization as an FST for cervical tumors larger than 2 cm [50].…”

“…Historically, a radical hysterectomy is defined by the excision of the adjacent parametrial tissue and upper vagina given risk of locally advanced disease. Compared to extrafascial (simple) hysterectomy, radical hysterectomy is associated with increased acute and chronic surgical complication including, but not limited to, genitourinary injury, fistula formation, bladder and bowel dysfunction [12,13].…”
