Cancer treatment is the most frequent cause of reduced fertility in cancer patients, with up to 80% of survivors affected. None of the established or experimental fertility preservation methods can assure parenthood; instead it may provide a future opportunity to overcome treatment induced sterility. Previous research demonstrated that fertility counselling has clinical and psychological benefit. Therefore, such patient services are recommended by internationally recognized guidelines. Around 70-75% of young cancer survivors in retrospective studies are reported to desire parenthood but the numbers of patients who use fertility preservation services prior treatment are significantly lower. Moreover, despite existing guidelines healthcare professionals worldwide lack practical knowledge and have personal biases which prevent addressing fertility preservation issues adequately. Surveys of healthcare professionals report the following barriers: lack of time and knowledge about existing options, poor prognosis, and delay in treatment, patient's age, partnership status, existing children, sexual orientation and socioeconomic situation. Moreover, fertility preservation consultation is not limited to medical aspects. Patient's fears, expectations and priorities shaped by personal values have to be addressed in a light of medical necessities, realistic survival prognosis, socio-cultural environment and availability of resources. We call for a need of framework for patient centred fertility counselling with a proposal that such framework should include support in decision making which would help patients to understand medical aspects of their cancer, realistic fertility preservation options, identify their preferences based on personal values and goals. Optional support services could also include legal guidance, psychological and spiritual support and financial counselling.

Cerebral germinoma is rare. Although the imaging of the germinoma is very evocative, it's very polymorphic clinical expression is at the origin of misguided diagnosis, as illustrated in our case. We report the case of a 10-year-old girl with diabetes insipidus evolving for 12 months associated with a decrease in visual acuity. Brain MRI (Magnetic Resonance Imaging) revealed a tumor process in the suprasellar region. Th e stereotaxic biopsy of the tumor confi rmed the diagnosis of the hypothalamic germinoma, which allowed the patient to be treated by radiotherapy and chemotherapy. Th e incidence of cerebral germinoma, its clinical (principally diabetes insipidus) and radiological features as well as therapeutic strategies are discussed hereby. Th e cerebral germinomas are germ-line tumors representing 4% of childhood brain tumors (Resetkova et al. 2013). We report a new observation of cerebral germinoma discovered through a polyuric polydipsic syndrome. Th e literature has been reviewed with discussion of the diagnosis, the radiological criteria, and the treatment of these tumors. Key words: diabetes insipidus, panhypopituitarism, intracerebral germinoma Subject and ResultsA 10-year-old girl was hospitalized in the pediatric neurology unit for drowsiness and cognitive disorders. She also complained of frontal headaches and progressive polyuric polydipsic syndrome (drinking between 3 and 7 liters per day), associated with growth failure. Medical history was unremarkable, except for a defi cit in Von Willebrandt factor detected during the pre-operative exams for adenoids. Th e clinical examination was normal; no sign of puberty was observed. A blood sample analysis showed a low free T4 (fT4) level at 7.4 pmol/l (reference 10.4-24.0 pmol/l) with normal t hyroid stimulating hormone (TSH) at 3.7 mU/l (reference 0.70-5.3 mU/l), increased level of prolactin at 63.3 μg/l (reference 5.0-23.0 μg/l), and low levels of gonadotropins. Plasma sodium and glucose were within the normal range. Th erefore, central hypothyroidism and diabetes insipidus (DI) were suspected. Urine osmolality was low at 88 mOsm/kg (reference 300-900 mOsm/kg), while plasma osmolality was normal at 281 mOsm/kg. A hydric deprivation test was not performed because the polyuric polydypsic syndrome was severe (3 to 7 liters per day). Th e concentration of anti-diuretic hormone (ADH) was low <0.5 pg/ml (reference 0.5-14 pg/ml) confi rming the DI. A glucagon test was performed (Table 1) to explore the anterior pituitary, there was no signifi cant increase of plasma cortisol and growth hormone (GH) levels aft er stimulation. Th e defi cit in fT4 and cortisol were treated by L-Th yroxine® 50 μg/day and hydrocortisone, while DI was successfully treated by Minirin®.

Despite breast cancer (BC) is a disease of postmenopausal women (the incidence gradually increases from 0.1/100,000 for women <20 years to 100-350/100,000 for women ≥70 years [ 1 ]), approximately 7 % of cases in the developed world and 25 % of patients in the developing world [ 2 ] are diagnosed in young women (i.e., <40 years). BC accounts for >40 % of all cancers in this age group (Table 3.1 ) and recent data show BC incidence in young women is increasing [ 3 ].Young age, even after adjustment for socio-demographic and tumor characteristics, is generally considered an independent predictor of poorer survival after BC [ 4 ]. In a series of 873 patients aged ≤45 years from 20 public data sets, proliferation gene signatures showed no signifi cant interaction with age in estrogen receptor positive/human epidermal growth factor receptor negative (ER+/HER2−) tumors but an inferior relapse-free survival was suggested in this subgroup as compared to women >40 years at diagnosis [ 2 ]. On the contrary, the outcome of 315 very young patients (<35 years at BC diagnosis) with Luminal A subtype who received adjuvant endocrine therapy (ET) was similar to that of older women [ 5 ].The increased number of long-term young BC survivors has focused the attention of healthcare professionals to long-term adverse effects of cancer therapies. In addition to the risk of heart failure and secondary neoplasms, oncologists must consider the impact of antineoplastic treatments on premature ovarian failure and, thus, on fertility.Young women with BC often face dilemmas about fertility, pregnancy, breastfeeding, and contraception.
