Objectives This study aims to evaluate the fracture resistance of endodontically treated anterior teeth restored with crowns made of composite or ceramic and retained without the use of a post (endocrowns) or with posts of 5 mm (short) and 10 mm in length (long). Material and methods Forty-eight intact maxillary incisors were selected for the study. After endodontic treatment, the crowns were sectioned 2 mm coronally to the cementoenamel junction provided with a ferrule of 2 mm. The roots were randomly divided into six groups (n=8) according to the post length and type of coronary restoration. The crowns were fabricated with the chairside economical restoration of esthetic ceramics system. Group 1 was restored with a 10-mm glass fiber post, composite core, and a full-coverage ceramic crown (LPCer); group 2, with a 5-mm glass fiber post, composite core, and a full-coverage ceramic crown (SPCer); group 3, with a 10-mm glass fiber post, composite core, and a fullcoverage composite crown (LPCpr); group 4, with a 5-mm glass fiber post, composite core, and a full-coverage composite crown (SPCpr); and groups 5 (EndoCer) and 6 (EndoCpr) were restored with ceramic and composite endocrowns, respectively. The teeth were then thermomechanically loaded in a chewing machine. After fatigue, the specimens were loaded to fracture. Data were analyzed with ANOVA and chi-square test. Mode of failure was defined as repairable or non-repairable. Results Presence of post, post length, and crown material had no significant effect on the fracture resistance. Groups restored with endocrowns presented a higher number of repairable fractures in respect to the other groups. Conclusions Presence of a post had no effect on the restorations' fracture strength. Clinical relevance Although this in vitro study has some limitations in respect to its clinical relevance, the restoration of largely destroyed anterior teeth with the use of an endocrown or a short glass fiber post might have advantages over a large glass fiber post.

This study investigated the effect of fiber reinforced composite resin (FRC) post lengths, ferrule, and full coverage metal crown on fracture resistance of endodontically treated teeth (ETT) using an experimental FRC post with the similar elastic modulus of dentin (11.5 GPa). ETT was restored according to 1 of 7 combinations of FRC post length (10, 5, 2.5, 0 mm), ferrule (0, 2 mm), and with/ without metal crown (n=6). An oblique compressive load was applied on the restored teeth using a universal testing machine. Fracture loads were evaluated with 1-way ANOVA and Tukey HSD test (p<0.05). Compared with tooth preparation only, the fracture loads of ETT restored with composite resin and different post lengths were not significantly different. The group with the ferrule with metal crown showed significantly greater fracture resistance than the others.

Post-Retained Single Crowns versus Fixed Dental Prostheses: A 7-Year Prospective Clinical Study

The objective of this prospective clinical trial was to assess the influence of the type of prosthetic restoration as well as the degree of hard-tissue loss on 7-year clinical performance of endodontically treated teeth restored with fiber posts. Two groups (n=60) were defined depending on the type of prosthetic restoration needed: (1) single unit porcelain-fused to metal (PFM) crowns (SCs) and (2) 3-4 unit PFM fixed dental prostheses (FDPs). Within each group, samples were randomly divided into 2 subgroups (n=30) according to the amount of residual coronal tissues after abutment build-up and final preparation: (A) more than 50% of coronal residual structure, (B) equal to or less than 50% of coronal residual structure. The clinical outcome was assessed based on clinical and intra-oral radiographic examinations at the recalls after 6, 12, 24, 36, 48 and 84 months. Data were analyzed by Kaplan-Meier Log Rank test and Cox regression analysis (p<0.05). The overall 7-year survival rate of endodontically treated premolars restored with fiber post and either PFM SCs or FDPs was 69.2%. The highest 84-months survival rate was recorded in Group 1A (90%), whereas teeth in Group 2B exhibited the lowest performance (56.7% survival rate). The Log Rank test detected statistically significant differences in survival rates among the groups (p=0.048).
