Femur shaft fractures are common fractures that orthopaedic surgeons come across. Such fractures frequently associate with high energy trauma (ex: Motor vehicle accident, fall from height) in young patients or a trivial fall in elderly patients. IMIL nailing is the most frequently used modality of management for shaft of femur fractures. Early definitive treatment within 24-48 hours in stable patients reduces the risk of mortality and morbidity. Fracture of femoral neck is an unusual complication while performing IMIL shaft fracture. We present a case of iatrogenic fracture neck of femur while performing IMIL for fracture shaft of femur and the technique of fixation used for the same intra-operatively. A 49-year-old man suffered a closed oblique fracture of the middle third of his left femur. Closed reduction and internal fixation by intramedullary (IM) nailing were performed. Per-operative fluoroscopic imaging and initial postoperative X-rays were judged normal and the patient followed the usual rehabilitation protocol. At 3-month follow-up the patient still demonstrated poor knee function and pain. A plain X-ray and a CT scan of the left knee revealed a displaced fracture of the medial femoral condyle. Analysis of the postoperative imaging suggests that the fracture occurred during the insertion of the IM nail. The nail possibly hit the Steinmann traction pin in the distal femur causing the medial condyle fracture. The patient was reoperated; open reduction and internal plate and screw fixation were performed with satisfactory clinical progress postoperatively. The description and illustration of this case is intended to make trauma surgeons aware of this rare but serious complication of IM femoral nailing.

Evidence from two studies (one RCT, one cohort) suggests that dynamization of intramedullary nailing (IMN) and static IMN for femoral shaft fractures appear to be comparable with regard to risk of nonunion, nail breakage, and limb shortening. Confl icting results with regard to mean time to union were noted. Additional methodologically rigorous comparative studies, which include patient-reported outcomes and larger sample sizes, are necessary to establish the long term safety and effi cacy of these two operative treatments.
