Femoral Deformity in Tibia Vara

Abstract: Patients with infantile tibia vara most commonly had normal alignment of the distal parts of the femora; substantial valgus deformity was not observed. Distal femoral varus constituted a substantial portion of the genu varum in children with late-onset disease. When correction of late-onset tibia vara is planned, the surgeon should be aware of the possibility that distal femoral varus is a substantial component of the deformity.

“…Mechanical lateral distal femoral angle (mLDFA) (normal range, 86° to 90°) and mechanical medial proximal tibial angle (mMPTA) (normal range, 85° to 89°) were measured on radiographs preoperatively to assess the contribution of the distal femur and proximal tibia in the origin of the knee varus deformity24). Preoperative MA and PTSA were recorded radiographically9).…”

“…The skeletal dysplasias, including achondroplasia, multiple epiphyseal dysplasia and spondyloepiphyseal dysplasia are more rare causes of lower extremity malalignment. Other rare causes of angular deformities in children include congenital longitudinal deficiency of the tibia and fibula, 3 focal fibrocartilaginous dysplasia, 4,5 and multiple hereditary exostoses. Traumatic and infectious causes include Cozen's fracture of the proximal tibial metaphysis, which can result in a temporary valgus deformity of the proximal tibia.…”

“…1 It is now believed that many other deformities accompanied tibial varus in this disease. 2,3 Proximal tibial procurvatum and internal torsion are among the other common deformities found in Blount disease. 2 Compensatory valgus deformity of the femur, once considered common in Blount disease, has been proven vice versa.…”

“…2 Compensatory valgus deformity of the femur, once considered common in Blount disease, has been proven vice versa. 3 Adding to the contradiction, distal femoral varus takes portion in causing genu varum in late onset-disease. 3 In line with the understanding of the deformities, managements of those deformities have also advanced.…”
