Aim: To evaluate sexual functions of women having stress urinary incontinence (SUI) preoperatively and 6 months and 12 months after undergoing a transobturator tape (TOT) ± pelvic organ prolapse (POP) surgery. Methods: One hundred-ninety-five women with SUI ± POP were recruited and 150 sexually active women who had clinical ± urodynamic SUI and underwent TOT ± POP surgery were included in this prospective study. Urogynecologic symptoms were evaluated preoperatively, at 6 months, and 12 months by Female Sexual Function Index (FSFI). Results: One hundred fifty women completed the study. Seventy-four underwent TOT-only and 76 underwent TOT + concomitant surgery. Mean total scores of FSFI were 21.7 ± 7.8, 22 ± 8.7, and 22.1 ± 8 in the preoperative period, postoperative 6, and 12 months, respectively. There was significant improvement in desire and total scores in the TOT-only group, whereas there was no significant difference in the TOT + concomitant surgery group except for significant worsening in the lubrication domain. The frequency of sexual intercourse increased while that of coital incontinence decreased after surgery. Conclusions: In our study, TOT was associated with decrease in coital incontinence and significant improvement in desire. In addition, there was significant improvement in FSFI desire and total scores in the TOT-only group, whereas no significant difference was observed in the TOT + concomitant surgery group except for the worsening of lubrication.

Considering that there is no effective treatment for human T-cell leukemia virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis, this study aimed to assess the impact of triple combination therapy-interferon-α, valproic acid, and prednisolone-on clinical outcomes, main HTLV-1 viral factors, and host anti-HTLV-1 antibody response. HTLV-1 proviral load (PVL), and HBZ and Tax mRNA expression levels were measured in peripheral blood mononuclear cells of 13 patients with HTLV-1-associated myelopathy/tropical spastic paraparesis before and after treatment with 180 μg pegylated interferon once a week, 10-20 mg/kg/day sodium valproate, and 5 mg/day prednisolone for 25 weeks using a TaqMan real-time polymerase chain reaction assay. Furthermore, anti-HTLV-1 titer, Osame Motor Disability Score, Ashworth spasticity scale, and urinary symptoms (through standard questionnaire and clinical monitoring) were assessed in patients before and after the treatment. HTLV-1 PVL and HBZ expression significantly decreased after the treatment [PVL from 1443±282 to 660±137 copies/10 4 peripheral blood mononuclear cells (p= 0.01); and HBZ from 8.0±1.5 to 3.0±0.66 (p<0.01)]. Tax mRNA expression decreased after the treatment from 2.26± 0.45 to 1.44±0.64, but this reduction was not statistically significant (p=0.10). Furthermore, anti-HTLV-1 titer reduced dramatically after the treatment, from 3123±395 to 815±239 (p<0.01). Clinical signs and symptoms, according to Osame Motor Disability Score and Ashworth score, improved significantly (both p<0.01). Urinary symptoms and sensory disturbances with lower back pain were reduced, though not to a statistically significant degree. Although signs and symptoms of spasticity were improved, frequent urination and urinary incontinence were not significantly affected by the triple therapy. The results provide new insight into the complicated conditions underlying HTLV-1-associated diseases.

OBJECTIVE:Stress urinary incontinence is one of the diseases that change the quality of life of women. Although there are various medical and surgical treatment modalities exist, transobturator tape surgery is one of the current options with the least complication rates. The aim of this systematic review was to evaluate sexual functional results after the transobturator tape procedure.STUDY DESIGN: Data were taken from those cohort, case-control and cross-sectional studies that were done with human subjects in full texts, were written in the English language. RESULTS:A total of 24 clinical trials were evaluated. The most frequently administered scales were the female sexual function index and The Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire -12. In total, 8 clinical studies employed female sexual function index whereas 8 clinical studies administered Prolapse/Urinary Incontinence Sexual Questionnaire -12. 4 of these studies tried to understand women's sexual functions through questions determined by the researchers (N/A). 2 of the other studies used the Sexual Impact Questionnaire and a visual analogue scale, 1 study used Nine Questions on Sexual Functioning (NSF-9) and 1 study used International Consultation on Incontinence Questionnaire Vaginal Symptoms Module (ICIQ-VS). It was found that transobturator tape surgery that is performed for incontinence produces positive results on both sexual functions and urinary functions. CONCLUSION:Maintaining urinary continence is detected to directly be correlated with sexual functions. It is of high importance that before initiating surgical treatments, clinicians should give the patients a detailed medical examination in terms of both urinary and sexual functions.
