Fecal Excretion of Calprotectin in Colorectal Cancer Relationship to Tumor Characteristics

Abstract: The majority of patients with colorectal cancer have increased fecal concentration of calprotectin. One single fecal spot seems to be sufficient for determination of the calprotectin level. Measurement of fecal calprotectin may possibly become of value as a marker for colorectal cancer, although calprotectin, similar to fecal occult blood (FOB) tests, is a non-specific test for colorectal pathology, also being elevated in inflammatory bowel diseases. Further investigation of its specificity is therefore needed. Show more

“…Since carcinoma cells do not synthesize calprotectin, this normal reading suggests that an exudative response had not been provoked. Nevertheless, the sensitivity and specificity of calprotectin in colorectal carcinoma were similar to those in previous reports [10,16,17]. The levels of calprotectin in patients who had colonic polyps or adenomas were within the normal range, presumably a reflection of the limited inflammatory response these polyps were producing within the colonic mucosa.…”

“…As in previous reports [14][15][16], there was no significant difference according to colonic site. One patient with ascending carcinoma colon had a normal level.…”

“…In contrast to previous studies [6], the difference between the diagnostic groups in the present study did not reach statistical significance. The median CPT value of 13 mg/l in relatives with CRC is lower than the value of 50 mg/l found in patients with already diagnosed CRC [6, 7, 10, 25, 26]. Possible explanations for this low value could either be bias because of the limited number of patients with CRC or that all the subjects with carcinoma were asymptomatic, in contrast to patients with previously known CRC in earlier studies.…”

“…17 Despite several advantages of calprotectin as a stool maker, fecal calprotectin finally revealed its critical pitfall in differentiating between inflammatory bowel diseases and cancer. 18,19 Furthermore, specificity is too low for screening of colonic polyps …”
