Trigeminal neuralgia (TN), the most common form of severe facial pain, may be confused with an ill-defined persistent idiopathic facial pain (PIFP). Facial pain is reviewed and a detailed discussion of TN and PIFP is presented. A possible cause for PIFP is proposed. (1) Methods: Databases were searched for articles related to facial pain, TN, and PIFP. Relevant articles were selected, and all systematic reviews and meta-analyses were included. (2) Discussion: The lifetime prevalence for TN is approximately 0.3% and for PIFP approximately 0.03%. TN is 15–20 times more common in persons with multiple sclerosis. Most cases of TN are caused by neurovascular compression, but a significant number are secondary to inflammation, tumor or trauma. The cause of PIFP remains unknown. Well-established TN treatment protocols include pharmacotherapy, neurotoxin denervation, peripheral nerve ablation, focused radiation, and microvascular decompression, with high rates of relief and varying degrees of adverse outcomes. No such protocols exist for PIFP. (3) Conclusion: PIFP may be confused with TN, but treatment possibilities differ greatly. Head and neck muscle myofascial pain syndrome is suggested as a possible cause of PIFP, a consideration that could open new approaches to treatment.

Aim of the research is to analyze the patterns of pain syndromes and their characteristics in multiple sclerosis patients. Materials and Methods. A total of 104 multiple sclerosis patients complaining of pain during the last month were examined in Lviv Regional Multiple Sclerosis Center. We analyzed medical records, performed general medical and neurological examinations and compiled history of lifestyle, illness and pain. The VAS and PainDetect questionnaires were used to assess the characteristics of pain syndromes. The patients were asked to choose one of the 4 typical patterns of pain, which best described their feelings. Then the analysis of pain syndrome patterns and their characteristics was carried out. Results and Discussions. All 4 typical patterns of pain syndromes were registered in multiple sclerosis patients. However, only 26.92% of the participants experienced an intermittent pain pattern. The rest of patients suffered from patterns characterized by persistent pain. The evaluation of the patterns and their characteristics revealed typical features. The highest intensity of the severest and average pain during the last month was present in the patients suffering from constant pain with distinct attacks: 8.0 [7.0; 9.0] points (p<0.05) and 7.0 [5.0; 8.0] points (p<0.05), respectively. Persistent paroxysmal pain was more common in the multiple sclerosis patients with neuropathic pain (43.8±8.8% vs 20.4±5.8%, p<0.05). Irradiating pain is also characteristic to this pattern in 52.9±8.6% of patients (p<0.05). The head is the most common pain localization in the patients experiencing persistent pain with severe attacks - 82.4±9.2% of cases (p<0.05). Conclusions. Pain is a permanent symptom in a significant number of multiple sclerosis patients. It can have different patterns of progression. However, it has typical characteristics and their understanding will aid in the diagnosis, prognosis and treatment of pain syndromes in multiple sclerosis patients

Актуальность. Одним из синдромов, на который чаще всего жалуются больные рассеянным склерозом (РС), является болевой синдром, который во многих случаях остается недооцененным как врачами, так и больными, однако в значительной степени влияет на качество их жизни. Цель: оценить характеристики качества жизни больных РС в зависимости от типа болевого синдрома. Материалы и методы. Обследовали 104 больных РС, которые имели жалобы на боль в течение последнего месяца. У каждого пациента определен тип болевого синдрома, проведен опрос с помощью стандартизированных опросников (Pain Detect, ВАШ, SF-36), после чего был проведен анализ характеристик качества жизни при различных типах боли. Результаты. У больных РС с болевыми синдромами низкие значения показателей физического и психологического компонента качества жизни обнаружены при всех типах боли. Частыми типами болевых синдромов были постоянные боли в конечности/конечностях, головная боль напряжения, болезненные тонические спазмы, боль при спастичности. Медианные показатели характеристики «боль» при всех типах боли были ниже 50 %, что демонстрирует влияние боли на формирование структуры качества жизни. Наиболее низкие значения психологического компонента качества жизни отмечались при болевых синдромах, характеризующихся регулярной внезапной высокоинтенсивной приступообразной болью, а именно феномене Лермитта — 31,1 % [24,4; 45,9], мигрени — 31,7 % [29,5; 39,1] и болезненных тонических спазмах — 35,4 % [28,0; 40,2]. Наиболее низкое значение физического компонента качества жизни было при скелетно-мышечной боли, индуцированной постуральными аномалиями, — 25,4 % [24,8; 28,1]. Наиболее низкое значение среди характеристик качества жизни имела характеристика «ролевые ограничения в связи с физическими проблемами» — 9 из 11 типов боли имели срединное значение 0 %. Большинство показателей шкал качества жизни имели более высокие значения у больных с одним типом боли, чем у больных с 2–3 типами (р < 0,05). Так, срединное значение физического компонента здоровья было достоверно выше на 18,8 % (р = 0,004) у больных с одним типом боли, чем у больных с 2–3 типами. В психологическом компоненте разница между группами составила 4 % (р = 0,05). Выводы. Болевые синдромы, их тип, характеристики, сочетание влияют на формирование структуры качества жизни больных РС. Адекватная коррекция болевых синдромов с учетом их типа и характеристик является дополнительным инструментом для улучшения качества жизни пациентов с РС. Наиболее низкие значения психологического компонента качества жизни отмечались при болевых синдромах, характеризующихся регулярной внезапной высокоинтенсивной приступообразной болью. Наиболее низкое значение физического компонента качества жизни было при скелетно-мышечной боли, индуцированной постуральными аномалиями. Наличие у больного РС нескольких типов боли в большей степени влияет на физический компонент качества жизни.
