Feasibility of Radical Transurethral Resection as Monotherapy for Selected Patients With Muscle Invasive Bladder Cancer

Abstract: Radical transurethral bladder tumor resection is a reliable therapeutic approach for patients with muscle invasive bladder cancer after complete tumor resection and with negative biopsies of the tumor bed. These results are consistent in all age ranges.

“…The 10-year cancer-specific survival was similar in these two groups and, furthermore, 57% of those managed with TUR alone survived with their bladders intact [Herr, 2001]. A prospective phase II study of 133 patients with muscleinvasive bladder cancer managed with radical TUR alone demonstrated a 79.5% overall survival and 64.9% bladder-intact survival at 10 years [Solsona et al 2010]. The key inclusion criteria for this trial were no residual tumor (pT0) upon repeat biopsy of the tumor bed and absence of hydronephrosis.…”

“…Em estadios II a IV (M0) pode-se considerar quimioterapia neoadjuvante com esquemas a base de Cisplatina, seguido por cistectomia radical e linfadenectomia estendida. Se for identificada doença com invasão da camada muscular profunda é indicado quimioterapia adjuvante por 4 ciclos após cistectomia radical, podendo ser substituída por radioterapia ou máxima ressecção transuretral e quimioterapia paliativa nos pacientes não candidatos à cirurgia (SOLSONA, et al, 2010).…”

“…Although these treatment options have fewer complications than a RC initially, the complications increase over time. Even in highly selected patient groups, both MIBC recurrence and tumor progression occur in 30% of patients with MIBC treated with TURBT alone [26] . BCG instillation therapy has a lower response in elderly patients and a higher rate of complications, such as bladder pain, voiding symptoms and fever [27,28] .…”
