Feasibility of Left Lobe Living Donor Liver Transplantation Between Adults: An 8-Year, Single-Center Experience of 107 Cases

Abstract: Operative mortality for a right lobe (RL) donor in adult living donor liver transplantation (LDLT) is estimated to be as high as 0.5-1%. To minimize the risk to the donor, left lobe (LL)-LDLT might be an ideal option in adult LDLT. The aim of the study was to assess the feasibility of LL-LDLT between adults based on a single-center experience of 107 LL-LDLTs performed over 8 years. The mean graft weight of LL grafts was 452 g, which amounted to 40.5% of the estimated standard liver volume of the recipients. Th… Show more

“…Despite the greater difficulty of hepatic artery reconstruction when selecting left hepatic grafts with multiple hepatic arteries, we prefer to use a left hepatic graft because it increases donor's safety (Nishizaki et al, 2001;Soejima et al, 2006;Taketomi et al, 2009). Table 1.…”

“…A simple way is to increase the graft volume by the application of right-lobe LDLT or auxiliary LDLT. 2,3 Another way is decompression of the excessive portal pressure and flow into a small graft. This includes the formation of a portocaval shunt or performing splenectomy or splenic artery ligation.…”

“…As we previously reported, our key GV/SLV value for the choice of graft is 35%. 2 If the GV/SLV is less than 35% in the left-lobe graft, the right-lobe graft is chosen. However, several cases in our series eventually had GV/SLV Ͻ 30% with a 1-year graft survival of 80%.…”

“…As is well known, the chance of receiving a liver graft from a deceased donor is almost zero in Japan. [7][8][9] The estimated autograft volume after ECHR was 590 g, representing a graft volume/standard liver volume ratio of 49.6% (Fig. 2).…”
