Feasibility of Certified Quality Management in a Comprehensive Stroke Care Network Using Telemedicine: STENO Project

Abstract: Conclusions Certified quality management within a large stroke network using telemedicine is possible and might improve stroke care procedures and thrombolysis rates. Outcome studies comparing conventional stroke care and telestroke care are inevitable.

“…For the telestroke service of STENO, a telemedicine unit (EVITA L1, ORI GmbH, Erlangen) was placed in one room of the emergency department, providing full audio-visual communication between NMGH and one of the hubs for acute stroke cases. Technical details and the organisation of STENO are described elsewhere in detail [13].…”

“…At the patient's site in the emergency room, the EVITA unit of the telestroke network was used. Data safety technology and regulations, therefore, were adopted from the STENO network [13]. Before this study, the telemedicine unit was already used for assessments in about 500 cases and also in an accompanying feasibility study with the same technical standards [14].…”

“…Отметим, что в некоторых странах теленеврологические сети сертифицируют по ISO 9001:2008. В частности, «телеинсульт» сеть Северной Баварии (Германия) демонстрирует рост числа тромболизисов и оптимизацию управления в течение 3 лет благодаря внедрению ме-неджмента качества 267 . Этими же авторами доказано, что методология «телеинсульта» не мо-жет быть эффективно применяться для иных направлений неврологической помощи.…”

“…A direct observation of the patient in their home with a dynamic live streaming of the patient's movements along with an audio line could give an exact idea of the patient's condition in their normal environment and thus provide a pertinent solution to their problem. Telemedical systems can meet these requirements by enabling remote video and audio live streaming and have already proven their efficacy in acute stroke care …”

“…Telemedical systems can meet these requirements by enabling remote video and audio live streaming and have already proven their efficacy in acute stroke care. 3 First attempts to use telemedicine for PD care date back to the 1990s, 4 but the video quality was not adequate. Larger studies in 2001 and 2004 with more than 150 participants had cohorts with general neurological conditions.…”
