Feasibility and acceptability of HIV self‐testing among pre‐exposure prophylaxis users in Kenya

Abstract: Introduction: HIV testing is key to the delivery of pre-exposure prophylaxis (PrEP): testing HIV-uninfected at-risk persons is the first step for PrEP initiation and ongoing HIV testing is an essential part of PrEP delivery. Thus, novel and cost-effective HIV-testing approaches to streamline delivery of PrEP are urgently needed. Within a demonstration project of PrEP for HIV prevention among high-risk HIV serodiscordant couples in Kenya (the Partners Demonstration Project), we conducted a pilot evaluation of H… Show more

“…This is not surprising as most people found the test instructions easy to understand and the test easy to perform. Other studies have also reported low usage of the helpline . Despite low usage, this feature may be worth retaining as it appeared to be most useful for study participants who tested positive in order to obtain support – over half the callers were those who tested positive, and it presents an important opportunity for referral to HIV treatment.…”

“…Eighty‐nine percent of clients with a baseline risk factor had a partner with unknown HIV status, highlighting that increasing partner testing could improve client risk assessment. Given the low cost of HIV self‐test kits ($2 as negotiated by the Bill and Melinda Gates Foundation), providing HIV self‐test kits to promote partner testing might be an efficient method for refining client decisions about PrEP . The utility of these strategies will depend on the how well risk can be evaluated by both client and provider.…”

“…These benefits foster opportunities for couples to discuss their sexual behavior, plans for pregnancy, concerns about HIV risk, and other topics that are sensitive but important for reducing HIV risk 17 . By designing this study under an implementation science framework, we had an opportunity to refine standardized messages for couples about biomedical HIV prevention and pilot add-on components that would increase efficiencies in intervention delivery, such as HIV self-testing, less frequent monitoring of kidney function, and adherence support through text messages 10 18 20 .…”
