FDG PET/CT in Malignant Eccrine Porocarcinoma Arising in a Pre-existing Poroma

Abstract: In a 61-year-old man with known poroma of right lower abdomen, malignant transformation to porocarcinoma was suspected and confirmed by biopsy. PET/CT was requested for tumor staging, which revealed high FDG uptake in the known skin nodules located in the right side of abdominal and chest wall and identified further some adenopathy in the right axillary. All nodules and axillary lymph nodes were removed, and diagnosis of eccrine porocarcinoma was confirmed. The primary tumor and secondary lesions of porocarcin… Show more

“…A second FDG-PET-CT five months later revealed lesions in skin, bone, lungs and lymph nodes. Another case was reported in 2007, also confirming correct diagnosis of metastases using FDG-PET-CT [18] and a similar third case was described in 2014 by Golemi et al [19]. The primary tumour as well as subsequent metastases display a high glucose metabolism, which probably explains adequate detection.…”

“…[1][2][3][4] High 18 F-FDG accumulation has been reported to be a prevalent feature of sweat gland malignancies. [5][6][7][8] However, in this case, the primary tumor and the metastatic lymph nodes had only a slight tracer uptake on 18 F-FDG PET/CT. These lesions showed intense 68 Ga-FAPI uptake on 68 Ga-FAPI PET/CT, which suggests that this subtype of sweat gland carcinoma may express a low level of glucose transporter protein type 1 and a high level of fibroblast-activated protein.…”

“…EPC usually arises on the lower extremities (44%), followed by the trunk (24%), head & neck (23%), upper extremities (11%), and rarely involves other areas [1] . To date, we found only seven cases of EPC that have been reported to arise on the abdomen, and all cases were greater than 60-years of age [11] , [12] , [13] , [14] , [15] , [16] . To the best of our knowledge, this case represents the first report of EPC arising on the abdomen of a patient with UC and in the early 20s.…”
