Objective: The relation between excessive prolactin and endometriosis-related infertility is debatable. Anovulation or defective luteal phase occurs frequently due to hyperprolactinemia in subfertile women. In this investigation, we evaluated the association between serum prolactin levels and the severity of endometriosis. Methods: This retrospective cohort study carried out at the Babol Infertility Research Center looked into the baseline serum prolactin levels of 114 infertile women with endometriosis and compared them to the levels seen in 101 infertile women without endometriosis (controls). Statistical analysis included independent t-test, chi-square, Welch test and ROC curve analysis. Results: Infertile women with endometriosis had significantly higher serum prolactin levels than infertile women without endometriosis ( p =0.003). A significant difference was detected between controls and individuals with endometriosis stages III/IV ( p -value=0.009). Prolactin was found to have diagnostic value to detect endometriosis stages III/IV vs. stages I/II in AUC=0.65, 95% CI (0.55, 0.76). Prolactin values with a cut off set at 20.08 ng/mL had a sensitivity of 0.74 and specificity of 0.54 in detecting disease stages III/IV vs. I/II. The prognostic capability of prolactin in detecting endometriosis in cases vs. controls by ROC curve analysis had an AUC=+0.67, 95% CI (0.60, 0.74). Prolactin values with a cut off set at 17.5 ng/mL had a sensitivity of 0.64 and specificity of 0.63 in segregating subjects with and without endometriosis. Conclusion: Higher prolactin levels were observed in infertile women with more severe endometriosis when compared to infertile women without endometriosis. Prolactin levels act as a probable prognostic biomarker to detect endometriosis stages III/IV vs. I/II and segregate infertile women with endometriosis from subjects without endometriosis.

Endometriosis is an incurable hormone-dependent inflammatory disease that causes chronic pelvic pain and infertility characterized by implantation and growth of endometrial tissue outside the uterine cavity. Symptoms have a major impact on the quality of life of patients resulting in socioeconomic, physical and psychological burdens. Although the immune system and environmental factors may play a role in the aetiology of endometriosis, oestrogen dependency is still considered a hallmark of the disorder. The impact of oestrogens such as oestrone and particularly, oestradiol, on the endometrium or endometriotic lesions may be mediated by steroids originating from ovarian steroidogenesis or local intra-tissue production (intracrinology) dependent upon the expression and activity of enzymes that regulate oestrogen biosynthesis and metabolism. Two key pathways have been implicated: while there is contradictory data on the participation of the aromatase enzyme (encoded by ), there is increasing evidence that the steroid sulphatase pathway plays a role in both the aetiology and pathology of endometriosis. In this review, we consider the evidence related to the pathways leading to oestrogen accumulation in endometriotic lesions and how this might inform the development of new therapeutic strategies to treat endometriosis without causing the undesirable side effects of current regimes that suppress ovarian hormone production.

Endometriosis is characterized by ectopic endometrial tissue and affects millions of women worldwide. The disease leads to various symptoms such as chronic pelvic pain and infertility and does not yet have a well-defined etiology. The pathology is similar to cancer, since endometrial cells are highly proliferative, invade tissues and may be associated with tumor suppressor genes, including p53. Genetic polymorphisms are responsible for phenotypic variations in the population and the development of endometriosis is due to a combination of multiple genes and environmental factors. The Arg72Pro polymorphism of the p53 gene alters the amino acid 72 from arginine to proline and studies indicate that there is a considerable increase in the risk of cancer in patients with this polymorphism, because the protein becomes more susceptible to degradation. The eNOS gene participates in angiogenesis, a necessary factor for endometrial cells to survive outside the uterus and has increased ©FUNPEC-RP www.funpecrp.com.br Genetics and Molecular Research 17 (3): gmr18046 T.R. Santos et al. 2 expression in patients with endometriosis during the menstrual cycle. The Glu298Asp eNOS polymorphism is due to a point mutation of glutamate to aspartate, which can generate changes in its enzymatic activity. Aspartate generates protein products with different susceptibility to cleavage, functionally affecting the protein. We detected the Arg72Pro polymorphism of p53 and the Glu298Asp polymorphism of eNOS and estimated their prevalence in fertile and infertile patients diagnosed with endometriosis. The techniques used were conventional PCR and ARMS-PCR. Patients with Pro or Asp polymorphic alleles were found to be more susceptible to the development of endometriosis, and an association of these two polymorphisms is directly linked to infertility. The target genes p53 and eNOS can be used as molecular markers for endometriosis diagnosis, guiding prognosis and treatment, and may contribute to a better understanding of the pathophysiology of endometriosis.
