Linfomas são transformações neoplásicas de células linfoides, o Linfoma Não-Hodgkin raramente ocorre na cavidade oral, os casos intraorais podem manifestar-se como inchaço, dor, dormência, mobilidade dentária ou linfadenopatia cervical, os achados radiográficos podem assemelhar-se a osteomielites, neoplasias secundárias ou lesões odontogênicas. Pode ocorrer aumento de volume em tecido mole e não cicatrização após extrações dentárias - sinais suspeitos de malignidade. Os linfomas são lesões malignas agressivas, com alto potencial de cura através de quimioterapia associada ou não à radioterapia, se diagnosticados precocemente. A cirurgia geralmente não é indicada. O prognóstico é bom para doenças localizadas, sendo menos favorável para doença disseminada. Cabe ao cirurgião-dentista diagnosticar manifestações estomatognáticas, contribuindo para o tratamento médico efetivo. O objetivo deste estudo é o relato de caso clínico de paciente com lesão osteolítica na mandíbula, com diagnóstico de Linfoma não-Hodgkin difuso de grandes células B a partir da investigação clínica multidisciplinar.

Background: This systematic review aimed to investigate the radiological features of lymphomas and leukaemias affecting the jaws.Methods: A systematic literature review was conducted using the electronic databases of PubMed, Web of Science and Scopus. Articles that contained sufficient radiographic examinations (periapical, panoramic or computed tomography images) for individual cases were included. Additionally, either immunohistochemical or molecular confirmation was required prior to inclusion. Three authors evaluated and described the image quality and radiological features.Results: From an initial 1079 articles screened, 129 cases were included, containing 88 tomographic, 76 panoramic and 26 periapical examinations. The quality of the majority of images was sufficient for evaluation. Diffuse large B-cell lymphoma, Burkitt lymphoma, leukemic infiltration, plasmablastic lymphoma and extranodal Natural killer (NK)/T-cell lymphoma, nasal type were the most common subtypes. Involved teeth presented with mobility in 37.2% of the cases and a provisional diagnosis of inflammatory/infectious dental disease was considered in 49.2% of cases. Computed tomography exams were available for 76% of the cases, with most presented with an osteolytic lesion with ill-defined borders. Periosteal reactions were uncommon. Conclusion:Lymphoma/leukaemia infiltrates of the jaw bones are usually of highgrade subtypes and rarely present with periosteal reactions.

BACKGROUND Diffuse large B-cell lymphoma (DLBCL) is the most common type of malignant lymphoma (ML), accounting for 30%-40% of cases of non-Hodgkin’s lymphoma (NHL) in adults. Primary paranasal sinus lymphoma is a rare presentation of extranodal NHL that accounts for only 0.17% of all lymphomas. ML from the maxillary sinus (MS) is a particularly rare presentation, and is thus often difficult to diagnose. We have reported the first known case of DLBCL originating from the MS with rapidly occurrent multiple skin metastasis. CASE SUMMARY An 81-year-old Japanese man visited our hospital due to continuous pain for 12 d in the left maxillary nerve area. His medical history included splenectomy due to a traffic injury, an old right cerebral infarction from when he was 74-years-old, hypertension, and type 2 diabetes mellitus. A plain head computed tomography (CT) scan revealed a 3 cm × 3.1 cm × 3 cm sized left MS. On day 25, left diplopia and ptosis occurred, and a follow-up CT on day 31 revealed the growth of the left MS mass. Based on an MS biopsy on day 50, we established a definitive diagnosis of DLBCL, non-germinal center B-cell-like originating from the left MS. The patient was admitted on day 62 due to rapid deterioration of his condition, and a plain CT scan revealed the further growth of the left MS mass, as well as multiple systemic metastasis, including of the skin. A skin biopsy on day 70 was found to be the same as that of the left MS mass. We notified the patient and his family of the disease, and they opted for palliative care, considering on his condition and age. The patient died on day 80. CONCLUSION This case suggests the need for careful, detailed examination, and for careful follow-up, when encountering patients presenting with a mass.
