Fatal Disseminated Toxoplasmosis During Primary HIV Infection

Abstract: Toxoplasmosis is a well recognized manifestation of AIDS, but the disseminated disease is a rare condition and it has not been associated to HIV seroconversion to our knowledge. We describe a fatal episode of disseminated T. gondii acute infection with massive organ involvement during primary HIV infection. The serological data demonstrate primary T. gondii infection. The avidity index for HIV antibodies supports recent HIV-1 infection.

“…Depending on social and cultural habits, geographic factors, climate, and transmission routes prevalence of this infection varies widely between different countries (10-80 %) (Robert-Gangneux and Darde 2012). The importance of toxoplasmosis mainly lies among pregnant women (due to the risk of transmission to fetus), transplant recipients and patients with acquired immunodeficiency syndrome (Derouin and Pelloux 2008;Signorini et al 2007;Sukthana 2006). The incidence of maternal infection during pregnancy is 1-8 per 1000 pregnancies.…”

“…T. gondii is an important cause of abortion in sheep and goats (Dubey and Beattie 1988). Toxoplasmosis also has a major public health concern because this disease is serious in terms of mortality or physical and/or psychological sequelae in patients with HIV disease and in congenitally infected neonates (Biesiada et al 2006;Signorini et al 2007).…”

“…But toxoplasmosis in immunocompromised individuals and the fetus can cause serious and fatal symptoms (Signorini et al, 2007;Torgerson & Mastroiacovo, 2013). Nowadays, the diagnosis of human toxoplasmosis is usually based on the detection of the anti T.-gondii antibodies by serological methods including ELISA, latex agglutination (LA), and indirect fluorescent antibody (IFAT) (RobertGangneux & Darde, 2012).…”

“…The disease in the immunocompetent adults and children are generally without symptoms or cover gentle and impulsively resolved signs; for example, fever, fatigue, lethargy and lymphadenopathy (Furtado, 2011;Mahmoudvand, 2016). On the other hand, toxoplasmosis in immunocompromised patients such as transplant recipients and patients with acquired immunodeficiency syndrome and the fetus can give rise to serious and fatal symptoms such as eye infection and brain involvements (Signorini et al, 2007;Torgerson & Mastroiacovo, 2013). The diagnosis of T. gondii infection is usually based on serological tests to find the anti-T. gondii antibodies; so that IgM and IgG anti-T. gondii antibodies are typically wellknown as a indicator of acute and chronic infection respectively (Hill & Dubey, 2002).…”
