BackgroundFascial Manipulation® (FM®) is a manual method of treatment created by Luigi Stecco that centres on the muscular fascia. Fascia is a fibrous collagenous tissue which, with its wide strands, connects all the connective tissues in the body into a whole. Fascia can also cause restriction of motion of the organs as a result of an injury, inflammation or infection. FM® technique bases on identification and therapy of specific points, which are located at the point of convergence of the vectorial forces of the muscles involved in every movement. The aim of this literature review was to analyze publications on the use of FM® in treatment of pain and functional dysfunctions.Materials and methodsMaterial for this literature review was found by two independent authors using PubMed, Cochrane library, ScienceDirect and PEDro databases. In order to identify appropriate studies, a combination of keywords ,,fascial manipulation”, „pain”, „dysfunction” was used. Finally, 10 publications were qualified for a literature review.ResultsIn the analyzed ten publications, improvement after treatment, reduction of pain and better functional outcome in patients conditions were figured. Six authors had some similar conclusions which state that the positive results of treatment last over time. One study presented opposite findings and state that the treatment has not brought long-lasting effects. ConclusionsConsidering all the reports on this subject, it is suggested that FM® therapy is worth considering in treatment of pain disorders, orthopaedic dysfunctions and postural disabilities. Further research of the subject on whether the treatment results are sustainable is necessary.

Background: Hip arthroplasty is considered the treatment of choice to improve the quality of life of patients affected by degenerative arthritis. The post-op rehabilitation regimen, however, is still a matter of debate. The goal of this study was to perform a systematic review of the available best evidence to provide recommendations for rehabilitation after hip arthroplasty. Materials and methods: Biomedical databases were accessed to identify guidelines, systematic reviews and randomised controlled trials addressing rehabilitation after hip arthroplasty published between 2004 and 2019. Studies were selected and extracted by two independent evaluators with standardised tools. Results: 1 guideline, 8 systematic reviews and 5 randomised controlled trials were included. All included papers were organised according the available evidence of clinical course chronology both in pre- and post-operation rehabilitation up to 6 weeks and thereafter. Although the value of a rehabilitation program after hip arthroplasty is universally recognised, the exact timing and number of sessions is still unknown. A solid literature review allows us to partially answer to this question. Conclusions: Evidence-based rehabilitation recommendations are proposed according to literature research findings. Clinical practice is still somewhat dependent on dogma and traditions, highlighting the need for additional high-quality clinical studies to address areas of uncertainty.

Background: Total knee arthroplasty (TKA) is one of the most commonly performed orthopedic surgeries in the lower extremity. However, patient dissatisfaction and functional disability are mostly experienced because of pain and limited range of motion (ROM). Although manual therapy is commonly implemented to improve ROM and modulate pain in the management of musculoskeletal disorders, there is a lack of evidence about its clinical effectiveness on postoperative TKA rehabilitation. Objective: To investigate the effectiveness of an exercise program combined with manual therapy compared with an exercise program only for pain, ROM, function, quality of life, and patient satisfaction outcomes. Design: A randomized controlled clinical trial. Setting: Rehabilitation unit of a university hospital. Participants: Forty-two patients (68.45 AE 6.3 years) scheduled for unilateral TKA as a treatment of severe osteoarthritis. Interventions: Joint and soft tissue mobilizations in addition to exercise therapy were provided to the mobilization group (n = 21) while the control group received exercise therapy only (n = 21). Main Outcome Measures: The outcome measures were numeric pain-rating scale, knee ROMs, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, 10-meter walk test (10MWT), 5-times sit to stand test (5SST), and Short Form-12 (SF-12). Results: Improvements in pain outcomes were significantly higher in the mobilization group than in the control group (P = .001, F [3, 33] = 7.06) and the between-group difference in change score was 1.3 points (P = .001). Although the between-group difference in change score was 12.8 for knee flexion at 2 months (P = .001), the overall group-by-time interactions were not significant for flexion and extension ROMs (P = .175, F [3, 33] = 1.75 and P = .57, F [2, 34] = 0.56, respectively). Additionally, there were statistically meaningful group-by-time interactions on total WOMAC score (P = .006, F [2, 34] = 5.29), 10MWT (P = .002, F [3, 33] = 5.98), and SF-12 mental component summary (P = .01, F [2, 34] = 4.92) favoring the mobilization group. Also, patient satisfaction was higher in the mobilization group (P = .02). Conclusions: A structured exercise program combined with manual therapy can be more beneficial in improving pain, function, and patient satisfaction compared to exercise program alone for postoperative TKA patients.
