Family history and socioeconomic factors as predictors of myocardial infarction, unstable angina and stroke in an Italian population

Abstract: A comprehensive case-control study was conducted in an Italian region in order to compare the influence of family history of cardiovascular events, socioeconomic factors, social networks, and their joint associations with major risk factors, on the risk, of myocardial infarction (MI), unstable angina (UA) and ischemic stroke (IS). A total of 513 patients with MI, 178 with UA, 237 with IS, and 928 hospitalised controls were recruited. The odds ratio (OR) of MI for two or more relatives with a positive history o… Show more

“…10,11,32,34 Two studies did report sex-of-parent effects for family history of MI in patients with MI, 23,24 but they were not population based, were restricted to younger age groups, and did not report sex-of-offspring/sex-ofparent interactions. One study attempted to determine sex-of-offspring/sex-of-parent interactions, 24 but it was a retrospective comparison of 2 separate single-sex cohorts in which data were collected by different methods.…”

“…Two studies that investigated the influence of environmental factors other than smoking on family history of stroke as a risk factor for stroke found no significant association. 35,64 Tenʈ of 12 studies 56,59 investigating a possible interaction between family history of stroke and hypertension in patients found a significant association; 3 studies found an association with smoking, 35,44,52 3 found an association with diabetes mellitus, 37,44,52 4 found an association with the presence of ischemic heart disease or vascular disease, 31,37,46,52 and 1 found an association with the presence of hyperlipidemia. 42 Two studies found that family history of stroke was significantly related to a cluster of vascular risk factors.…”

“…Two case-control studies 46,64 and 1 prospective cohort study 26 reported the effect if both parents had been affected by stroke 26,64 or vascular disease. 46 The combined OR for a family history of stroke in the case-control studies increased significantly (Pϭ0.018) when they were analyzed separately for a family history in only 1 parent (OR, 1.08; 95% CI, 0.7 to 1.6; Pϭ0.77; heterogeneity Pϭ0.35) versus a family history of both parents (OR, 2.45; 95% CI, 1.4 to 4.2; Pϭ0.002; heterogeneity Pϭ0.…”
