Families' Importance in Nursing Care

BackgroundIn Quebec (Canada), nearly 20,000 pregnancies end in miscarriage, and the majority of the miscarriages are dealt with in an emergency unit. Although there are studies documenting the effects of this type of grief on mental health, men’s experiences are much less discussed than those of women. Similarly, no study has evaluated best practices in terms of service continuity, from emergency care to community resources. The aim of this study is to better understand the relationships that exist between the organization of emergency room and primary care health services for women presenting with miscarriage, on the one hand, and the positions and experiences of women and men within these services, on the other.MethodsThe general objective of this mixed-method study can be broken down into three methodological sections. Focus 1. Institutional discourses and practices. This section is structured as a multiple case study of the mandates of five participant institutions. The study will involve (a) a documentary analysis; (b) a quantitative survey (N: 200) and (c) group interviews (N: 75) with caregivers and emergency unit managers. Focus 2. Women’s and men’s experiences of miscarriages and the institutional response. This section includes (a) a survey (N: 232) and (b) individual interviews (N: 80) designed to identify best practices in emergency involving women and their partners in each area. Focus 3. This section will integrate the information furnished by the first two sections in order to create an ethnographic overview of the situation.DiscussionThis innovative project will provide answers to critical questions on how to improve the effectiveness and quality of interdisciplinary and multisectoral interventions to promote the mental health and psychosocial well-being of couples having experienced a miscarriage. It will have a material effect on the organization of emergency services and of the primary care pathway for women experiencing a miscarriage and for their partners.Trial registrationNot applicable. This study involves a retrospective view of usual health care interventions. This study is not a clinical trial that prospectively assigns human participants or groups of humans to one or more health-related interventions to evaluate the effects on health outcomes.

Aims To identify, appraise and describe the characteristics and measurement properties of instruments assessing nurses’ attitudes towards the importance of involving families in their clinical practice. Design Psychometric systematic review. Data Sources The PubMed, CINAHL, PsycINFO, Web of Science, and Cochrane Library databases were searched from their time of inception to June 2018. Review Methods Two reviewers independently assessed the eligibility of studies, extracted data, and appraised the methodological quality of the studies using the COSMIN checklist. Results A total of 19 studies representing 5 instruments were included. Two instruments—a revised version of the Families’ Importance in Nursing Care: Nurses’ Attitudes scale (FINC‐NA‐R) and the Family Nurse Practice Scale (FNPS)—obtained higher scores for most of their properties. The instruments used in the remaining eight studies require further assessment of their psychometric properties. Conclusion This psychometric review offers a synthesis of the quality of the available instruments and gives a better understanding of nursing theory‐based tools. The FNPS and FINC‐NA‐R instruments seem to be the most suitable measures of nurses’ attitudes towards the importance of involving families in their clinical practice. Impact The growing emphasis on improving nurse–family relationships and the quality of care provided makes it necessary to have valid and reliable instruments for assessing nurses’ attitudes towards the importance of involving families in their clinical practice. This paper provides evidence that will help clinicians and researchers make decisions about the most suitable instrument based on a critical appraisal and comparison of the measurement properties according to a rigorous methodology.

Zusammenfassung Hintergrund und Fragestellung Frühgeborene Kinder brauchen oft längere Betreuung auf einer Neonatologie und sind daher von Eltern getrennt. Förderliche und hinderliche Faktoren für den Eltern-Kind-Bindungsaufbau aus Sicht der Eltern und der Fachpersonen sind kaum beschrieben. Methoden Dafür wurde eine qualitative Studie in einem universitären Zentrumsspital der Schweiz mit betroffenen Eltern und Fachpersonen durchführt. Die Eltern (7 Frauen, 2 Männer) wurden mittels Einzel- und Zweierinterviews durchgeführt. Den Fachpersonen (n = 125) wurde der Fragebogen FINC-na online zum Ausfüllen zugesendet. Zusätzlich wurden Interviews mit ausgewählten Fachpersonen durchgeführt (12 Frauen, 2 Männer). Transkribierte Interviewdaten wurden inhaltlich und thematisch analysiert. Soziodemografische und FINC-na-Daten wurden mit R statistisch behandelt. Ergebnisse Es wurden n = 9 einzelne Eltern (7 Frauen, 2 Männer) befragt. Dabei zeigte sich, dass die Eltern ihre Rolle finden müssen und sich fühlen, als lebten sie in zwei Welten. Fast die Hälfte der angeschriebenen Fachpersonen (45 %) füllten die Online-Version des FINC-na komplett aus. Zusätzlich wurden n = 14 Personen (12 Frauen, 2 Männer) in Einzel- und Gruppeninterviews befragt. Für die Fachpersonen war der Umgang mit anwesenden Eltern herausfordernd, insbesondere bei knappen personellen Ressourcen. Schlussfolgerung und Fazit für die Praxis Der Gesundheitszustand des Kindes und strukturelle Gegebenheiten beeinflussen den Bindungsaufbau. Auf Neonatologien sollten Möglichkeiten für körperliche elterliche Nähe regelmäßig und oft gefunden werden. Ebenso ist nötig, dass die Eltern in der selbständigen Versorgung des Kindes gefördert werden. Dabei ist es unumgänglich, dass Fachpersonen eine positive Haltung zum Einbezug der Eltern entwickeln.
