Familial Monozygotic Twinning: Report of an Extended Multi-generation Family

Abstract: T hirteen sets of monozygotic (MZ) twins from an extended multi-generation family are reported. Zygosity was determined by interviewing families for overall physical similarity and by assessment of facial photographs. A hypothetical gene was traced back five generations to a common grandfather. Familial monozygotic twinning in this pedigree is compatible with autosomal dominant inheritance with reduced penetrance. Other plausible mechanisms of inheritance are discussed.

“…There are few previous reports of FMZT [Harvey et al, 1977; Hamamy et al, 2004], totaling only 11 families. There are at least three possible reasons for this: (1) many MZ twin pairs are falsely diagnosed as DZ because they are not “identical”; (2) DC twins are usually assumed to be DZ, and; (3) in the context of FMZT, MZ families would be assumed to be DZ because familial MZ twinning is widely believed to be rare in comparison with familial DZ twinning, or not to exist at all.…”

“…It is generally assumed that, in contrast to dizygotic (DZ) twinning, monozygotic (MZ) twinning is seldom, if ever, familial. Two previous reports have given details of 11 familial MZ twinning (FMZT) pedigrees [Harvey et al, 1977; Hamamy et al, 2004].…”

“…Monozygotic (MZ) twinning has a very different etiology than DZ twinning [Hall, 2003]. MZ twinning does not seem to be influenced by genetic factors, maternal age and parity, though families with a history of MZ twinning have been reported [Bulmer, 1970; Hamamy et al, 2004]. Recently, an increase in MZ twin births has been reported after IVF and OI [Steinman, 2003; Derom et al, 2006].…”
