Vitamin D is an essential hormone for calcium gut absorption. It is also involved in child growth, cancer prevention, immune system responses, and tooth formation. Due to inadequate vitamin D intake and/or decreased sunlight exposure, vitamin D deficiency has resurfaced in developed countries despite known inexpensive and effective preventive methods. Vitamin D deficiency is a common cause of rickets, a condition that affects bone development in children and that can have serious dental complications. Deficiency during pregnancy can cause enamel hypoplasia of primary teeth. Enamel regeneration is currently impossible; hypoplasia is therefore irreversible, and once affected, teeth are prone to fast caries development. Deficiency during early childhood can affect permanent teeth and ensuing caries can sometimes lead to tooth loss at a young age. Oral manifestations of rickets should be diagnosed early by both physicians and dentists to prevent severe dental complications. This case study presents 2 young girls with rickets in early childhood who suffered from subsequent serious tooth decay.

Familiaire hypofosfatemische rachitis heeft als symptomen groeivertraging, vorming van O-of X-benen, gewrichtspijnen, multipele spontane odontogene abcesvorming en vertraagde eruptie van gebitselementen. De dentale symptomen zijn voornamelijk te wijten aan demineralisatie van dentine. Zonder adequate preventieve maatregelen kan familiaire hypofosfatemische rachitis leiden tot spontane pulpanecrose. Het aanbrengen van glazuurverzegelingen zou de pulpanecrose en dus de abcesvorming kunnen voorkomen. AnamneseDe medische voorgeschiedenis van de patiënt omvatte onder andere een chirurgische correctie van zijn O-benen in 1993 en 2006. In 2002 had zich nefrocalcinose, neerslag van kalk in de nieren waardoor nierstenen kunnen ontstaan, als complicatie van de behandeling ontwikkeld. In 2007 werd door mutatieanalyse een genmutatie van het vermoede verantwoordelijke gen vastgesteld en werd de klinische diagnose X-gebonden hypofosfatemische rachitis bevestigd. Vanwege gewrichtspijnen en het bemoeilijkte looppatroon stond de patiënt onder langdurige en intensieve begeleiding van onder andere een fysiotherapeut, een ergotherapeut en verschillende medische specialisten. Een belangrijk deel van de tijd was de patiënt afhankelijk van een rolstoel. De familieanamnese toonde dat de moeder en de opa van de patiënt met dezelfde aandoening waren belast, zij het bij de moeder in mindere mate.Als medicatie voor de hypofosfatemische rachitis was al op 1,5-jarige leeftijd begonnen met het voorschrijven van fosfaatdrank en alfacalcidol, een actieve vorm van vitamine D. Voor de gewrichtsklachten was de selectieve cyclo-oxygenase-2 remmer etoricoxib voorgeschreven. Door een ziekenhuistandarts was als cariësprofylaxe een mondspoeling van 0,05% natriumfluoride voorgeschreven. De naleving van deze medicatie was matig; vooral de fosfaatdrank werd vanaf de pubertijd niet altijd ingenomen. Als verklaring voor deze gebrekkige naleving noemde de patiënt de bittere smaak van de fosfaatdrank en het gebrek aan direct merkbaar effect van de behandeling.De voorgeschiedenis van de mondgezondheid toonde het herhaaldelijk ontstaan van dentale abcessen op jonge leeftijd. Op 3-jarige leeftijd had zich al een eerste abces rond de centrale incisieven in de bovenkaak ontwikkeld. Sindsdien waren 3 tot 4 abcessen per jaar ontstaan, waarvan een enkele thuis met behulp van keukengerei was ontlast. Enkele tijdelijke gebitselementen waren geëxtraheerd in verband met abcessen. Er was geen vertraagde eruptie van de blijvende gebitselementen opgetreden. Na doorbraak van de blijvende dentitie was de vorming van abcessen afgenomen. Sinds 2005 waren er intermitterende progressieve pijnklachten rond gebitselement 37 en tot 2010 was een enkele cariëslaesie gerestaureerd. Mineralization Defects in the Primary Dentition Associated With X‐Linked Hypophosphatemic Rickets

X -linked hypophosphatemia rickets (XLH) is caused by inactivating mutations in the phosphate-regulating endopeptidase homolog, X-linked (PHEX) gene, leading to renal phosphate wasting and hypophosphatemia. Dental mineralized tissues are affected by XLH, though tissue-specific dental mineralization defects have been variably described. We aimed to quantify dental effects in primary teeth of six pediatric patients with XLH (four females and two males, aged 5-12 years). All participants had received conventional treatment of calcitriol and phosphate from initial diagnosis. High-resolution μCT and histology were employed to analyze 15 exfoliated or extracted primary teeth from patients in comparison with primary control teeth from healthy children. Compared with controls, a third of the teeth from patients with XLH showed reduced enamel density, though enamel thickness was not reduced. Teeth from patients with XLH featured 10% reduction in mean dentin density, though teeth from three affected individuals had dentin densities within normal limits. Dentin mineralization defects were found across all regions of dentin in affected teeth: outer mantle dentin, circumpulpal dentin, and the most recently formed proximal pulpal dentin. Both crown and root dentin thickness appeared reduced in XLH-affected versus control teeth, with root dentin more severely affected. Interglobular dentin accumulation throughout the circumpulpal dentin was evident by both μCT and histology (585-fold volume increase and 45-fold area increase, respectively). Histology also revealed substantially wider predentin and decreased acellular cementum thickness in XLH-affected versus control teeth, though thinner cementum appeared to include functionally oriented Sharpey's fibers except in the most severely affected individuals. These data provide quantitative analyses of dental mineralization defects associated with XLH, finding a wide range of severity, with dentin more significantly and consistently defective across the spectrum of XLH.
