Falling through the cracks? Missed opportunities for earlier HIV diagnosis in a New York City Hospital

Abstract: Summary Newly diagnosed HIV-positive patients have frequent health care encounters prior to diagnosis representing missed opportunities for diagnosis. This study determines the proportion of patients with new HIV diagnoses with encounters in the 3 years prior to diagnosis. We describe the characteristics of newly diagnosed patients and of "late testers" (CD4 <200 cells/mm(3) at the time of diagnosis). We identified all newly diagnosed with HIV in emergency department, inpatient, and outpatient settings between… Show more

“…Studies have shown that multiple missed opportunities for HIV testing are prevalent in health-care settings, particularly in primary care clinics and emergency departments. 38,39 These missed opportunities result in late diagnoses and low CD41 counts. A study conducted in a New York City hospital found that 60% of 253 newly diagnosed HIV patients had made at least one hospital visit within three years prior to diagnosis, with a median of three visits.…”

“…This finding was established among reported partners whose HIV status was considered unknown or seronegative; the group that the local HIV program targets for HIV testing of partners. The limited identification of HIV-free partners and of undiagnosed HIV individuals imply missed opportunities for active HIV prevention [ 19 , 21 ] and for controlling morbidity rates of HIV among PLWH expected to be earlier diagnosed [ 22 24 ].…”

“…Such IC visits were related to pregnancy care, hospital admissions or routine care for HAART treatment, thus pointing to missed opportunities across health services offered at the study center. At a time when care of PLWH involves an increasing number of health services, strategic HIV prevention should be fostered beyond HIV programs involving key health services, particularly for the pool of individuals at high-risk, such as partners of PLWH [ 19 , 21 , 34 , 35 ].…”
