Failures in Thoracic Spinal Fusions and Their Management

Abstract: Instrumented fixation and fusion of the thoracic spine present distinct challenges and complications including pseudarthrosis and junctional kyphosis. When complications arise, morbidity to the patient can be significant, involving neurologic injury, failure of instrumentation constructs, as well as iatrogenic spinal deformity. Causes of fusion failure are multifactorial, and incompletely understood. Most likely, a diverse set of biomechanical and biologic factors are at the heart of failures. Revision surgery… Show more

“…Many current options for revision surgery to correct spine pseudoarthrosis carry high failure rates and risk of developing junctional kyphosis. 7 Pedicled VBGs possess robust blood supply and reduce morbidity and operative time associated with free tissue transfer and avoid additional skin incisions. 4 Although there are no absolute contraindications specific to pedicled VBGs compared with free bone flaps, relative common contraindications include chronic infections, diabetes, immunosuppression, substance abuse, previous surgery or irradiation at the harvest site, and inadequate soft-tissue coverage.…”

“…Additionally, according to a large meta-analysis by Prablek et al 33) , the incidence of pseudoarthrosis in thoracic fusion is approximately 1.8%. Therefore, it is necessary to consider the deterioration of OLF due to excessive stress on the upper and lower adjacent joints.…”
