Failure mode and effect analysis‐based quality assurance for dynamic MLC tracking systems

Abstract: FMEA proved to be a powerful and flexible tool to develop and implement a quality management (QM) framework for DMLC tracking. The authors conclude that the use of FMEA-based QM ensures efficient allocation of clinical resources because the most critical failure modes receive the most attention. It is expected that the set of guidelines proposed here will serve as a living document that is updated with the accumulation of progressively more intrainstitutional and interinstitutional experience with DMLC trackin… Show more

“…In contrast to the end‐to‐end test quality assurance procedures, this system identifies the contribution of the different components, and highlights particularly the components that have the largest uncertainties contributions which can be fixed or reduced in the specific clinical application. Furthermore, this approach may be useful in failure mode and effect analysis used by forthcoming AAPM Task Group‐100, 24 , 25 in which the procedure is broken into different processes and each process is divided into subprocesses where the failure modes and large uncertainty sources can be identified and quantified. Once a process or subprocess with frequent failure modes or large errors is identified, integrated quality control procedures can be implemented whereby more frequent checking on this particular process is performed to achieve accurate and safe operation.…”

“…Second, many reported radiation therapy incidences resulted from incorrect or inappropriate use of radiation treatment devices due to miscommunication or misunderstanding rather than device failures. As such, the development of quality management programs that focuses on the design and execution of various radiotherapy processes has become a subject of significant interest in recent years 12, 13, 14, 15, 16, 17, 18, 19, 20. Following the recommendations from the AAPM task group report No.…”

“…Data have been published on FMEA for stereotactic radiosurgery (SRS),8, 9, 10 stereotactic body radiation therapy (SBRT),11, 12 tomotherapy,13 intraoperative radiation therapy (IORT),14 treatment planning,15 and dynamic MLC tracking 16. These valuable publications primarily come from large institutions where staffing levels may allow for variable amounts of time allocation for projects of this magnitude.…”
