OBJECTIVE: Little is known regarding how providers should use information about intimate partner violence (IPV) to care for depressed patients. Our objective was to explore what depressed IPV survivors believe about the relationship between abuse, mental health, and physical symptoms and to elicit their recommendations for addressing depression. DESIGN: Focus group study. PATIENTS/PARTICIPANTS: Adult, English-speaking, female, Internal Medicine clinic patients with depressive symptoms and a history of IPV. INTERVENTIONS: Thematic analysis using an inductive approach (consistent with grounded theory), at a semantic level, with an essentialist paradigm. MEASUREMENTS AND MAIN RESULTS: Twenty three women participated in 5 focus groups. Although selected because of their depression, participants often felt their greatest concerns were physical. They acknowledged that their abuse history, depression, and physical complaints compound each other. They appreciated the need for health care workers to know about their depression and IPV history to get a "full picture" of their health, but they were often hesitant to discuss such issues with providers because of their fear that such information would make providers think their symptoms were "all in their head" or would encourage providers to discount their pain. Participants discussed difficulties related to trust and control in relationships with providers and gave recommendations as to how providers can earn their trust. CONCLUSIONS: Understanding a patient's IPV history may allow providers to develop a better therapeutic relationship. To treat depression adequately, it is important for providers to reassure patients that they believe their physical symptoms; to communicate respect for patients' intelligence, experience, and complexity; and to share control.KEY WORDS: intimate partner violence; depression; pain; physical symptoms; qualitative research; physician-patient relationship.

À la suite d'une démarche de planification socio sanitaire visant à définir les priorités d'action pour les années à venir, la violence conjugale est devenue une des deux problématiques prioritaires pour le CLSC Saint-Hubert et son dépistage, un objectif important du plan opérationnel. Pour atteindre cet objectif, le CLSC a fait appel au Centre de recherche interdisciplinaire sur la violence familiale et la violence faite aux femmes (CRI-VIFF). Une équipe de recherche de ce centre a travaillé à l'élaboration d'un protocole de dépistage systématique de la violence conjugale. Cet article d écrit l'élaboration de ce protocole et son application par un nombre limité d'intervenants et d'intervenantes auprès d'une partie de la clientèle du CLSC. Cette phase expérimentale précède l'application généralisée du protocole dans tous les services du CLSC.The issue of marital violence was chosen as one of two priorities by the CLSC Saint-Hubert staff and board following a broad-based community planning effort which was to guide actions for the coming years. Identifying women suffering from their husband's violent behavior became a primary goal. In order to achieve this goal, researcher's assistance was enlisted. Members of the Centre de recherche interdisciplinaire sur la violence familiale et la violence faite aux femmes (CRI-VIFF) developed a structured protocol to screen systematically adult female clients for spousal violence. This paper describes the development of the screening tool and its preexperimental utilization by several types of health and mental health professionals serving a segment of the CLSC Saint-Hubert population. This exploratory phase takes place prior to the generalization of the screening tool use to the CLSC entire service delivery System. To identify demographic and health indicators of domestic violence. Design: Anonymous questionnaire survey of patients regarding violence and a chart review regarding symptoms and diagnoses. Setting: Community-based family practice residency training center in a midwestern city. Participants: Women 18 years of age or older visiting the center over a 2-month period in 1990. Of 476 eligible participants, 394 (82.7%) consented to complete the survey. Measures: A detailed, standardized measure of violence was used. Physical and psychological problems were given codes from the International Classification of Diseases, Ninth Revision (ICD-9). Results: Younger women and those separated or divorced from their partners were more likely to have been victims. Never-married women also had substantially high rates of victimization. Depression was the strongest indicator of victimization, even when controlling for demographic factors. Back pain, ulcers, headaches, and anxiety were not strong indicators of abuse. A classification analysis showed that a combination of all variables could predict lifetime injury only about half the time and violence in the past year only about 20% of the time. Conclusions: Since neither demographic nor health factors could accurately predict who had been victimized, all women need to be asked about abuse. Physicians should also keep in mind that divorced and unmarried women are often affected by abuse, either immediately or by its l ong\ x=r eq-\ term aftereffects.
