Factors influencing quality of life in patients with active tuberculosis

Abstract: Background: With effective treatment strategies, the focus of tuberculosis (TB) management has shifted from the prevention of mortality to the avoidance of morbidity. As such, there should be an increased focus on quality of life (QoL) experienced by individuals being treated for TB. The objective of our study was to identify areas of QoL that are affected by active TB using focus groups and individual interviews.

“…The long treatment period was poorly understood by patients [23,26,28,38–40,46,52]; and adherence appeared to be facilitated where patients understood the importance of completing treatment [24,26,32,36,39,44,55,58,59]. One study on adherence to prophylaxis reported that nonadherent patients had little information on TB as a disease, but were very aware of the potential adverse effects caused by treatment [44].…”

“…Patients' beliefs about the efficacy of treatment, both positive [39,41,52,59] and negative [22,23,26,28,32,34,36,39,44,52,54–56], may impact on adherence. Patients may question the efficacy of the pills or think that only injections are “medicine” [22], or even question the validity of diagnostic tests that are not considered sophisticated enough for such a dangerous disease [52].…”

“…Although previous studies suggested that discrimination experience was negatively related to perceived QOL [7,11,17,18], it is still unclear whether effects of discrimination experience on QOL would be mediated by some individual's cognitive variables, such as the premigratory expectation or aspiration. The theory of goal-striving stress (TGSS) proposes that discrepancy between a migrant's premigratory aspiration and postmigration achievement (e.g., goal-striving stress or unfulfilled aspiration) is the source of mental distress [33].…”

“…21 Also, most of the respondents in this study group were married (61.6%) and this can be compared to a study done in North India which showed that about two-thirds of the participants were married among many other earlier studies which also supported the same finding. 21,25,26 About three-fifth of the respondents in this study were Christians; this is adjudged to be closely linked with the predominance of both Yoruba and Ibo tribes in the study population and more reflective of Christianity as the more predominant religion among both ethnic groups. More than half of the respondents earn income lower than 40,000 naira ($200) monthly and about two-third earned below the national monthly minimum wage of 20,000 naira ($100); which would have constituted a great problem to the control of this communicable disease.…”

“…Similar findings have been observed on the influence of age, sex, marital status and income on the QoL of people with TB. 26 More over a cross-sectional study in Estonia found similar socio-demographic factors (including age under 30 years, female gender and being employed) to significantly increased QoL.…”
