Factors influencing place of delivery for pastoralist women in Kenya: a qualitative study

Abstract: BackgroundKenya’s high maternal mortality ratio can be partly explained by the low proportion of women delivering in health facilities attended by skilled birth attendants (SBAs). Many women continue to give birth at home attended by family members or traditional birth attendants (TBAs). This is particularly true for pastoralist women in Laikipia and Samburu counties, Kenya. This paper investigates the socio-demographic factors and cultural beliefs and practices that influence place of delivery for these pasto… Show more

“…There is a need for awareness creation in the community at large, to achieve the Ethiopia reproductive health programme goal by 2020. Our study is in line with the findings from other pastoral communities showing that pastoral women prefer traditional birth attendants and healers and only travel to health facilities for complicated cases . Lack of privacy is mentioned as one of the most underreported but most important reasons for not using skilled delivery service (27% (95%CI 22.8–31.1%).…”

“…Studies have found that agro-pastoralist communities in remote areas have difficulty accessing FHFs resulting in the communities patronizing informal health facilities (IHFs) (Duba et al, 2001;Caulfield et al, 2016). However, Cheikhyoussef et al (2011) argue that traditional knowledge of plant species as medicines and traditional views on causes of diseases compel agro-pastoralists to rely more on traditional treatments.…”

Pastoralist women in Kenya continue to face long distances to care, posing a significant barrier and a threat related to obstetric emergencies, as seen in other studies (Caulfield et al, 2016); (Byrne et al, 2016). Some studies suggest construction of maternity waiting homes as a solution to distance, especially in pastoralist communities, but cultural resistance continues to be a challenge to the uptake of such interventions (Karanja et al, 2018).…”
