Factors influencing physical activity and rehabilitation in survivors of critical illness: a systematic review of quantitative and qualitative studies

Abstract: The main barriers identified were patient physical and psychological capability to perform physical activity, safety concerns, lack of leadership and ICU culture of mobility, lack of interprofessional communication, expertise and knowledge, and lack of staffing/equipment and funding to provide rehabilitation programs. Barriers and enablers are multidimensional and span diverse factors. The majority of these barriers are modifiable and can be targeted in future clinical practice.

“…Actigraphy has been used to measure activity in several patient groups, but not in patients recently discharged from an ICU . There is some evidential support regarding the positive effect of mobilization in the ICU, but the optimal duration and length of this intense rehabilitation are unknown. Unfortunately, we were not able to make any firm conclusions regarding the positive or negative effect of being physically more active in the ward.…”

“…Such barriers include: institutional issues (e.g. staffing, consultation/referral requirements for rehabilitation clinicians), clinician issues (e.g., training and inter-professional communication), and patient issues (details below) [6,7]. Barriers are often modifiable, particularly at the institution and clinician level [6,7].…”

“…Modern lifestyles promote sedentary behavior to a greater extent than PA (e.g., by requiring computer work), and many employees report lack of time as one of the main reasons for not performing PA [19,20]. Therefore, many researchers have begun exploring interventions that incorporate PA into work and other activities.…”
