Introduction: Adolescent idiopathic scoliosis is a common type of scoliosis among adolescents. A significant problem is the coexistence of depressive and anxiety disorders. However, the causes of this condition are not fully recognized. Thus, it seems reasonable to search for possible risk factors that may trigger mental disorder occurrence. Material and methods: This is a literature narrative review concentrating on the influence of the adolescent idiopathic scoliosis on the development of depression and anxiety disorders among adolescents, as well as possible therapeutic interventions. Google Scholar, Medline, PubMed and Science Direct databases were searched throughout January 2024 for relevant original and review articles from 2000 to 2024, using the following keywords: “adolescent idiopathic scoliosis”, “anxiety”, “body image”, “bracing”, “chronic disease”, “depression”, “mental disorders”, “pain”, “peer relationship”, “physical activity”, “pulmonary function”, “sleep disorders”, “surgery”, “treatment”. Manuscripts in other language than Polish or English were excluded from the search. To ensure the appropriate quality of this review, the Scale for the Assessment of Narrative Review Articles was used. Results: Chronic pain, sleep disorders, pulmonary function impairment, perception of the disease as a chronic somatic disease, disturbed body image, lack of social interactions, physical activity limitation and applied methods of treatment may lead to the depressive and anxiety disorders development in the adolescent idiopathic scoliosis course. Conclusions: Biological and psychosocial factors contribute to the pathogenesis of depressive and anxiety disorders among patients with adolescent idiopathic scoliosis. Their identification should help create a comprehensive plan of treatment, which may effectively maximize therapeutic outcomes in the context of both scoliosis and mental disorders. ResumoA escoliose idiopática é caracterizada por um quadro de deformidade tridimensional da coluna vertebral com rotação axial e inclinação lateral com angulação maior que 10° segundo o Método de Cobb. Sua abordagem pode se dar de maneira conservadora ou cirúrgica, a depender do grau de angulação, desenvolvimento osteomuscular e idade da criança ou adolescente acometido, ou ainda, a depender do comprometimento funcional advindo da condição. O objetivo deste estudo foi analisar o impacto da videotoracoscopia na abordagem da escoliose idiopática. Trata-se de uma revisão sistemática de literatura, construída conforme protocolo Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) e registrada no PROSPERO sob número CRD42022351466. A busca por estudos foi realizada entre agosto de 2022 e setembro do mesmo ano, em bancos de dados bibliográficos incluindo MEDLINE, The Cochrane Library e Web of Science. A técnica é uma alternativa minimamente invasiva à toracotomia que apresentou grande evolução nos últimos anos. Destacam-se como principais vantagens a menor perda sanguínea durante procedimento, menor tempo de internação e melhora da estética pós-cirúrgica. Uma das desvantagens citadas pelos autores é a maior complexidade e exigência tecnológica, maior tempo cirúrgico e necessidade de seleção criteriosa dos pacientes, conforme graus de desvio vertebral. O uso de analgésicos e anti-inflamatórios não teve diferença significativa entre os procedimentos tradicionais (toracotomia) ou videotoracoscopia.

The mediation effect of vitamin A and vitamin D supplement in the association between serum vitamin K levels and musculoskeletal disorders in preschool children

IntroductionVitamin K deficiency may elevate the incidence of musculoskeletal disorders (MSD), whereas it lacks validation for pediatric populations and has uncertain dose recommendations. In this context, we hypothesized that serum vitamin K levels are associated with MSD in preschool children, and the widely used vitamin A and vitamin D supplements may mediate these associations based on potential mechanisms, which expects to provide guidance for future practice.MethodsA cross-sectional study was conducted in Sichuan province in southwestern China, from January 2021 to May 2022. Serum levels of vitamin K1/K2 and 25(OH)D were determined using the high-performance liquid chromatography method, and the diagnosis of MSD was executed by clinicians. Overall and stratified logistic regression analysis based on categorized 25(OH)D levels were conducted to assess association between serum vitamin K levels and MSD prevalence after adjusting for confounders. Mediation analysis was further performed and vitamin A and D supplementation was regressed as the mediator.ResultsA total of 6,368 children aged 0–6 years old were enrolled. MSD was identified in 1179 (18.51%) of the children, while 5,189 (81.49%) of them did not present such disorder. After adjusting confounders, a significant difference was found in serum vitamin K1 level between children in MSD and Non-MSD group (OR = 0.802, 95%CI 0.745–0.864). No significant difference was found in serum vitamin K2 level between the two groups (OR = 0.975, 95%CI 0.753–1.261). The association between vitamin K1 level and MSD prevalence was partly (36.8%) mediated by vitamin A and D supplementation.ConclusionsA low serum vitamin K1 level is connected with an increased risk of MSD among children, highlighting that vitamin A and D supplementation is a helpful intervention to prevent MSD in children with vitamin K deficiency.
