Aim Percutaneous transluminal angioplasty or cephalic vein transposition to the axillary vein is mainly used for treatment of cephalic arch stenosis, a common complication of brachiocephalic fistulas. However, the results of such interventions have been disappointing. Methods We used a polytetrafluoroethylene prosthesis with a 6 mm diameter to bridge the cephalic vein and the ipsilateral external jugular vein, and successfully created a new drainage outlet and established immediate restoration of flow through brachiocephalic fistulas. Results This surgery allowed the cephalic venous arch and subclavian vein, which are vulnerable to stenosis, to be bypassed altogether and the puncture segment could be elongated by about 20 cm. Conclusion It is a safe and effective alternative to traditional methods of treatment for cephalic arche stenosis.

Bu araştırmanın amacı, 2012-2021 yılları arasında hemşirelik alanında yayımlanmış hemodiyaliz ile ilgili araştırmaların eğilimlerinin ve popüler noktalarının belirlenmesidir. Gereç ve Yöntemler: Araştırmanın verileri, Temmuz 2022 tarihinde Web of Science Core Collection veri tabanından toplanmıştır. Analizler CiteSpace 6.1.R3 programı ile yapılmıştır. Analizlerde yazarlar, atıf yapılan yazarlar, ülkeler, kurumlar, atıf yapılan dergiler ve referanslar ve anahtar kelimelerin ortak atıf, ortak oluşum ağı, küme ve atıf patlaması analizi yapılmıştır. Bulgular: Araştırmamıza 961 orijinal araştırma dâhil edilmiştir. Hemşirelik alanında hemodiyaliz araştırmaları 343 yazar ve 59 ülkeden 542 kurum tarafından gerçekleştirilmiştir. Bu araştırma alanının en üretken yazarı, Ana Luisa Brandao de Carvalho Lira'dır (n=12). En üretken ülke ve kurum ise Amerika Birleşik Devletleri (n=240) ve Sao Paulo Üniversitesidir (n=24). En fazla hemodiyaliz araştırmaları kabul eden dergi Nephrology Nursing Journal'dır (n=204). Hemodiyaliz araştırmalarında en fazla kullanılan ilk 5 anahtar kelime ise kronik böbrek hastalığı, yaşam kalitesi, hemodiyaliz, diyaliz ve böbrek hastalığı evresidir. Anahtar kelimelerin küme analizinde, en büyük ilk 3 küme intradiyalitik hipotansiyon, ev diyalizi ve hemodiyaliz tedavisi olarak bulunmuştur. Sonuç: Araştırmamızın sonucunda, bu alanın araştırma eğilimlerinin ve popüler noktalarının, kronik böbrek hastalığı, yaşam kalitesi, hemodiyaliz, diyaliz ve böbrek hastalığı evresi anahtar kelimeleri olduğu ve intradiyalitik hipotansiyon, ev diyalizi ve hemodiyaliz tedavisi konuları olduğu görülmüştür. Araştırma bulgularımızın, araştırmacılara, sağlık profesyonellerine ve kurumlara iş birliği yapabileceği araştırmacılar, ülkeler ya da kurumlar bulmasına yardımcı olabileceği düşünülmektedir. Ayrıca gelecekteki çalışmaların daha az çalışılan uyum, deneyim, yönetim, depresyon, hemodiyaliz hemşireleri, yakın akraba, nitel çalışma gibi konuları incelemesinin, bu alandaki hemşirelik bakımının ilerlemesine ve literatürünün derinleşmesine katkıda bulunacağı düşünülmektedir. Introduction: Arteriovenous fistula (AVF) is a primary dialysis vascular access commonly used for maintaining hemodialysis (MHD) patients. Vitamin D (VD) is a fat-soluble steroid hormone that is closely related to vascular endothelial function. This study aimed to investigate the association between VD metabolites and AVF failure in patients undergoing HD.Methods: This study included 443 HD patients using AVF between January 2010 and January 2020. The AVF operations in these patients were newly created by the same physician. We analyzed the AVF patency rates using the chi-square test. Univariate and multivariate logistic regression analyses were performed to explore risk factors for AVF failure. Survival analysis was performed to explore AVF survival at different serum 25-hydroxyvitamin D (25(OH)D) concentrations.Results: Logistic regression analyses showed that male sex; age; BMI; serum albumin, triglyceride, phosphorus, 25(OH)D, iPTH and hemoglobin levels, history of hypertension, CHD, diabetes, stroke, and antiplatelet drug use; and smoking habits were not risk factors for AVF failure. The failure incidence rates of AVF in subjects in the VD deficiency and non VD deficiency group were not statistically significant (25.0% vs. 30.8%, p = 0.344). The AVF failure incidence rates at 1, 3, and 5 years in the patients with 25(OH)D levels more than 20 ng/mL were 26%, 29%, and 37%, respectively, and the one-year AVF failure incidence rates were 27% in the patients with 25(OH)D levels less than 20 ng/mL. In addition, the Kaplan-Meier analysis suggested that the no significant differences were noted when calculating the cumulative survival rates of AVF between the two groups within 50 months of AVF using. Conclusion:Our findings suggest that 25(OH)D deficiency is not associated with AVF failure incidence rates, and that 25(OH)D deficiency has no significant impact on long-term cumulative AVF survival rate.
