Objective: To evaluate the impact of asthma on activities of daily living and on health status in patients with controlled, partially controlled, or uncontrolled asthma in Brazil. Methods: We used data related to 400 patients in four Brazilian cities (São Paulo, Rio de Janeiro, Salvador, and Curitiba), obtained in a survey conducted throughout Latin America in 2011. All study subjects were > 12 years of age and completed a standardized questionnaire in face-to-face interviews. The questions addressed asthma control, hospitalizations, emergency room visits, and school/work absenteeism, as well as the impact of asthma on the quality of life, sleep, and leisure. The level of asthma control was determined in accordance with the Global Initiative for Asthma criteria. Results: Among the 400 respondents, asthma was controlled in 37 (9.3%), partially controlled in 226 (56.5%), and uncontrolled in 137 (34.2%). The numbers of patients with uncontrolled or partially controlled asthma who visited the emergency room, who were hospitalized, and who missed school/work were higher than were those of patients with controlled asthma (p = 0.001, p = 0.05, and p = 0.01, respectively). Among those with uncontrolled asthma, the impact of the disease on activities of daily living, sleep, social activities, and normal physical exertion was greater than it was among those with controlled or partially controlled asthma (p < 0.001). Conclusions:In Brazil, asthma treatment should be monitored more closely in order to increase treatment adherence and, consequently, the level of asthma control, which can improve patient quality of life and minimize the negative impact of the disease.Keywords: Asthma/epidemiology; Quality of life; Hospitalization. ResumoObjetivo: Avaliar o impacto da asma nas atividades da vida diária e na saúde em pacientes com asma controlada, parcialmente controlada ou não controlada no Brasil. Métodos: Foram utilizados dados de 400 pacientes de quatro cidades brasileiras (São Paulo, Rio de Janeiro, Curitiba e Salvador) obtidos em um inquérito realizado em países da América Latina em 2011. Todos os indivíduos do estudo tinham idade > 12 anos e responderam a um questionário padronizado por meio de entrevista presencial. As questões abordavam o controle da asma, número de hospitalizações, de consultas de urgência, absenteísmo na escola/trabalho e impacto da asma na qualidade de vida, sono e lazer. OBJECTIVE:To evaluate inhaler technique in outpatients with asthma and to determine associations between the correctness of that technique and the level of asthma control.METHODS:This was a cross-sectional study involving patients > 14 years of age with physician-diagnosed asthma. The patients were recruited from the Asthma Outpatient Clinic of the Hospital de Clínicas de Porto Alegre, in the city of Porto Alegre, Brazil. The patients completed two questionnaires (a general questionnaire and an asthma control questionnaire based on the 2011 Global Initiative for Asthma guidelines), demonstrated their inhaler technique, and performed pulmonary function tests. Incorrect inhaler technique was defined as the incorrect execution of at least two of the predefined steps.RESULTS:We included 268 patients. Of those, 81 (30.2%) showed incorrect inhaler technique, which was associated with poor asthma control (p = 0.002). Logistic regression analysis identified the following factors associated with incorrect inhaler technique: being widowed (OR = 5.01; 95% CI, 1.74-14.41; p = 0.003); using metered dose inhalers (OR = 1.58; 95% CI, 1.35-1.85; p < 0.001); having a monthly family income < 3 times the minimum wage (OR = 2.67; 95% CI, 1.35-1.85; p = 0.008), and having > 2 comorbidities (OR = 3.80; 95% CI, 1.03-14.02; p = 0.045).CONCLUSIONS:In the sample studied, incorrect inhaler technique was associated with poor asthma control. Widowhood, use of metered dose inhalers, low socioeconomic level, and the presence of > 2 comorbidities were associated with incorrect inhaler technique.

Introdução: A asma é uma das principais causas de morbidade na infância, ocasionando grandes prejuízos aos pacientes. O Programa Respirar foi implantado na cidade de Ipatinga, localizada na região do leste mineiro, em abril de 2003 visando ao atendimento às crianças e adolescentes asmáticos com indicação do uso de anti-inflamatório inalado profilático. O objetivo deste estudo foi avaliar o controle da asma dos pacientes inseridos no Programa Respirar. Métodos: estudo longitudinal retrospectivo, com 305 pacientes portadores de asma com indicação de corticoterapia inalatória, com idades entre sete e 19 anos selecionados aleatoriamente no Programa Respirar da cidade em questão, mediante análise de prontuários referentes a janeiro de 2012 a dezembro de 2015. A análise estatística foi realizada utilizando os programas EpiData (3.03) e SPSS (19.0) e para a análise descritiva, tabelas de distribuição de frequência e medidas de tendência central (média) e de variabilidade (desvio padrão, mínimo e máximo). Para a comparação das variáveis clínicas no início e após a entrada no programa, foi aplicado o teste de McNemar no caso de variáveis categóricas e teste T-pareado para as variáveis numéricas. Em todas as análises, foi considerado nível de significância de 5%. Resultados: Quando comparadas as variáveis clínicas antes e depois de três anos de entrada no Programa Respirar, 51% dos pacientes melhoraram a classificação da asma, 60,1% das crianças diminuíram o número de hospitalizações por crise de asma e 86,7% reduziram o número de idas ao pronto-atendimento por crises de asma. Essas diferenças foram estatisticamente significativas (valor de p < 0,001). Conclusão: Os pacientes assistidos pelo Programa Respirar mantêm melhor controle sobre as crises de asma, reduzindo significativamente as hospitalizações e idas ao pronto-socorro por decorrência da doença.
