Factors Associated With Successful Completion of the Chronic Disease Self-Management Program by Adults With Type 2 Diabetes

Abstract: This study examines factors associated with completion (attendance ≥4 of 6 sessions) of the Chronic Disease Self-Management Program (CDSMP) by adults with type 2 diabetes. Patients with glycated hemoglobin ≥ 7.5 within 6 months were enrolled and completed self-report measures on demographics, health status, and self-care (n = 146). Significant differences in completion status were found for several self-care factors including healthful eating plan, spacing carbohydrates, frequent exercise, and general health. … Show more

“…This study took place in the context of an implementation analysis of the V1SAGES project, described elsewhere ( 10 ), aimed at implementing and evaluating a 6-month case management intervention by a nurse and the CDSMP in 4 ( 4 ) family medicine groups (FMG), including 38 practices, in the Saguenay-Lac-Saint-Jean region of the Province of Quebec (Canada). A FMG is an administrative arrangement for existing practices in which primary care physicians are grouped together to collaborate with nurses to offer primary care services to a group of registered patients.…”

“…A few studies on CDSMP using quantitative ( 3–7 ) and mixed ( 8 ) methods examining factors associated with completion (attendance greater than or equal to four out of six sessions), documented that participants reporting depression were less likely to complete the program ( 3 ), while completion rates for participants aged 75 years or more were higher than for younger participants ( 8 ). Participants who perceived better health were more inclined to complete the CDSMP ( 4 ). Completion was not influenced by race/ethnicity or socio-economics ( 4 ).…”

“…According to the health belief model (Rosenstock, ), the high degree of perceived threat of CVD complications, the combination of perceived susceptibility and severity and learning the benefit of preventive measures will strengthen the opportunity of the patients to perform and adhere to preventive behaviours. It is the role of healthcare providers to assess the barriers to preventive health behaviour and provide counselling accordingly to optimize self‐care (Helduser et al, ).…”

“…As shown in Table 2, DSMP is a 6-week group program conducted by trained community health workers (CHWs) and consists of diabetes education, self-management, action planning and problem solving, symptom management, and working with family and HCP. The DSMP has been demonstrated to significantly reduce depression, increase communication with physicians, promote healthy eating, and decrease hypoglycemia at 6 and 12 months (Erdem and Korda, 2014, Lorig et al, 2009), but has shown less conclusive evidence on improving clinic attendance (Helduser et al, 2013). Clinical navigation, on the other hand, has been shown to facilitate appointment setting and return visits, improve goal setting, and enhance some self-management (medication taking, blood glucose testing) (Hargraves et al, 2012), but does not improve key psychosocial or other self-management activities (Freund et al, 2008).…”
